摘　　要：社区获得性肺炎(CAP)是常见的呼吸道感染疾病之一。近年来,随着病原体变迁、混合感染日益增加、耐药菌增加、全球老龄化、合并慢性疾病等相关因素的影响,CAP的发病率逐年上升。目前,西医对于CAP主要采取抗感染治疗,但抗生素滥用导致的耐药菌增加,菌株变异、多重细菌感染等问题突出,因此正确选择抗菌药物、减少耐药发生、降低非重症肺炎进展为危重症肺炎风险、降低再住院率成为当前临床医师面临的重大挑战。而中医药在应对抗生素滥用和耐药方面有一定优势,临床可选用经方(麻杏石甘汤、清金化痰汤、小青龙汤等)、经验方、中医外治法(如穴位敷贴)等治疗CAP,且均已取得一定进展。未来可挖掘更多用于治疗CAP的经典名方,以为临床治疗CAP提供参考。Community-acquired pneumonia(CAP)is one of the most common respiratory infections.In recent years,the incidence of CAP has increased year by year due to related factors such as pathogen changes,increasing co-infection,increasing drug-resistant bacteria,global aging,and comorbid chronic diseases.At present,anti-infection treatment is mainly adopted for CAP in western medicine,but the increase of drug-resistant bacteria caused by antibiotic abuse,strain variation,multiple bacterial infections and other problems are prominent.Therefore,the correct selection of antibiotics,reducing the occurrence of drug resistance,reducing the risk of non-severe pneumonia progression to critical pneumonia,and reducing the rate of re-hospitalization have become a major challenge faced by the clinicians.Traditional Chinese medicine(TCM)has certain advantages in dealing with antibiotic abuse and drug resistance.TCM can be used to treat CAP clinically,such as TCM classic prescriptions(Maxingshigan decoction,Qingjin Huatan decoction,Xiaoqinglong decoction,etc.),empirical prescriptions,and TCM external treatment methods(such as acupoint application),and they all had made certain progress.In the future,more classic prescriptions for treating CAP can be discovered to provide references for the clinical treatment of CAP.
