摘　　要：目的:探讨CD38联合CD138在慢性子宫内膜炎(CE)合并不孕症诊断中的应用价值。方法:选取2023年3月—2023年4月于浙江省人民医院生殖医学中心进行体外受精-胚胎移植的120例不孕症患者作为研究对象,均实施宫腔镜检查并获取病理组织,进行病理染色及免疫组化染色,以病理检查为“金标准”,分析CD38联合CD138诊断CE的效能,统计CE与非CE患者CD38、CD138阳性率及妊娠率。结果:病理检查显示,120例不孕症患者确诊CE 74例。CD38联合CD138诊断CE的灵敏度为71.62%(53/74),特异度为84.78%(39/46),准确度为76.67%(92/120)。CD38联合CD138与病理检查诊断结果的一致性一般(Kappa=0.714,P<0.001)。CE组CD38^(+)CD138^(-)、CD38^(+)CD138^(+)占比高于非CE组,妊娠率低于非CE组(P<0.05)。CE组不同CD38、CD138阳性表达患者的妊娠率比较:CD38^(+)CD138^(+)>CD38^(-)CD138^(+)>CD38^(+)CD138^(-)(P<0.05)。结论:CD38联合CD138在CE合并不孕症诊断中的应用价值一般,与病理检查诊断结果的一致性一般。Objective:To explore the application value of CD38 combined with CD138 in the diagnosis of chronic endometritis(CE)complicated with infertility.Methods:A total of 120 infertility patients who underwent in vitro fertilization-embryo transfer at the Reproductive Medicine Center of Zhejiang Provincial People's Hospital from March 2023 to April 2023 were selected as the study subjects.All the patients underwent hysteroscopy.The pathological tissues were obtained for pathological staining and immunohistochemical staining.The pathological examination served as the"gold standard"to analyze the efficacy of CD38 combined with CD138 in the diagnosis of CE.The positive rates of CD38 and CD138 and pregnancy rates in CE and non-CE patients were counted.Results:Pathological examination showed that 74 of the 120 infertility patients were diagnosed with CE.The sensitivity,specificity,and accuracy of CD38 combined with CD138 in the diagnosis of CE were 71.62%(53/74),84.78%(39/46),and 76.67%(92/120).The consistency of diagnosis results between CD38 combined with CD138 and pathological examination was general(Kappa=0.714,P<0.001).The proportions of CD38^(+)CD138^(-)and CD38^(+)CD138 in the CE group were higher than those in the non-CE group,and the pregnancy rate in the CE group was lower than that in the non-CE group(P<0.05).The pregnancy rates of different CD38 and CD138 positive expressions in the CE group in the CE group were compared as follows:CD38^(+)CD138>CD38^(-)CD138^(+)->CD38^(+)CD138(P<0.05).Conclusion:CD38 combined with CD138 has high application value in the diagnosis of infertility complicated with CE,and has general consistence with the diagnosis results of pathological examination,and can predict the pregnancy rate of CE patients.
