基于孙申田教授经络辨证思想采用电项针结合手指点穴治疗颈性眩晕的临床研究

摘　　要：目的:观察电项针和手指点穴推拿方法治疗颈性眩晕的临床效果。方法:选取2023年6月—2024年1月黑龙江中医药大学附属第二医院推拿四科门诊及住院处就诊并同意入组的颈性眩晕患者60例,根据就诊先后顺序编号,采用随机数表法,随机分成治疗组和对照组,每组30例。对照组患者颈部仅采用牵引法联合颈部电针,而治疗组患者在此基础上加用电项针联合手指点穴治疗;比较两组患者治疗前后的颈椎病临床评价量表(CASCS)、疼痛视觉模拟评分(VAS)、颅内多普勒血流图(TCD)中基地动脉BA数值,并观察两组患者治疗前后血清炎性因子IL-6的含量变化情况。结果:治疗后,治疗组临床总有效率明显高于对照组(P<0.05)。两组治疗后CASCS评分均显著升高(P<0.05),且治疗组优于对照组,差异具有统计学意义(P<0.05)。两组治疗后VAS评分均下降(P<0.05),且治疗组低于对照组(P<0.01);两组椎基底动脉的血流数值有所提高(P<0.05),且治疗组优于对照组,差异具有统计学意义(P<0.01)。两组患者的血清IL-6均明显下降(P<0.05),其中治疗组效果明显优于对照组(P<0.01)。结论:电项针联合手指点穴推拿手法治疗颈性眩晕患者疗效显著,能更快地缓解患者痛苦,改善患者生活质量。Objective:To observe the clinical effects of electroacupuncture at the nape combined with finger acupressure massage in treating cervical vertigo.Methods:A total of 60 patients with cervical vertigo who agreed to be enrolled in the fourth Department of Massage and inpatient Department of the Second Affiliated Hospital of Heilongjiang University of Chinese Medicine from June 2023 to January 2024 were selected.According to the order of treatment,they were randomly divided into treatment group and control group by random number table method,with 30 cases in each group.The control group received neck traction combined with electroacupuncture to the neck,while the treatment group received electroacupuncture at the nape combined with finger acupressure.The cervical spondylosis clinical assessment scale(CASCS),pain visual analog scale(VAS),and transcranial Doppler(TCD)measurement of the basilar artery(BA)values were compared before and after treatment in both groups.Additionally,changes in serum inflammatory factor IL-6 levels were observed.Results:After treatment,the total clinical effective rate in the treatment group was significantly higher than that in the control group(P<0.05).VAS scores decreased in both groups after treatment(P<0.05),with the treatment group showing significantly lower scores than the control group(P<0.01).CASCS scores increased significantly in both groups after treatment(P<0.05),with the treatment group outperforming the control group(P<0.01).Serum IL-6 levels significantly decreased in both groups(P<0.05),with a greater reduction in the treatment group compared to the control group(P<0.01).Blood flow values of the vertebrobasilar artery increased in both groups(P<0.05),and the treatment group showed significantly better improvement than the control group(P<0.01).Conclusion:Electroacupuncture at the nape combined with finger acupressure massage provides significant clinical efficacy in treating cervical vertigo,offering quicker pain relief and improved quality of life for patients.
