宣肺化痰散瘀汤治疗支气管扩张伴感染(痰热壅肺证)的效果评价

摘　　要：目的:探究宣肺化痰散瘀汤治疗支气管扩张伴感染患者的临床疗效。方法:选取2020年10月—2023年6月在中国人民解放军南部战区海军第二医院82例支气管扩张伴感染患者,按信封抽签法随机分为对照组和观察组,每组41例。对照组进行常规西医治疗,观察组在对照组基础上加用宣肺化痰散瘀汤治疗。比较两组治疗前后的临床疗效、中医证候积分、肺功能指标、炎症指标、圣·乔治呼吸问卷(SGRQ)评分及安全性指标。结果:治疗后观察组总有效率显著高于对照组( χ^(2)=3.998,P=0.046)。治疗后观察组中医证候积分明显低于对照组(P<0.05)。观察组的FVC、FEV1均高于对照组(P<0.05),WBC、IL-6、LT-B4和hs-CRP水平均显著低于对照组(P<0.05)。观察组SGRQ各项评分明显低于对照组(P<0.05)。两组患者在治疗过程中没有出现显著的药物不良反应(P>0.05)。结论:结合宣肺化痰散瘀汤与标准西医疗法治疗支气管扩张并发感染(痰热壅肺证),能够显著提升患者的肺功能与免疫能力,并有效降低炎症指标水平,为该病证的复合治疗方案提供有效的治疗选择。Objective:To investigate the clinical efficacy of Xuanfei Huatan Sanyu Decoction in treating bronchiectasis patients with infection.Methods:A total of 82 patients with bronchiectasis and infection,treated at the Second Hospital of the Navy,Southern Theater Command of the Chinese People s Liberation Army,from October 2020 to June 2023,were randomly divided into a control group and an observation group,with 41 patients in each group.The control group received conventional Western medical treatment,while the observation group received Xuanfei Huatan Sanyu Decoction in addition to the treatment given to the control group.Clinical efficacy,TCM syndrome scores,lung function parameters,inflammatory markers,St.George s Respiratory Questionnaire(SGRQ)scores,and safety indicators were compared between the two groups before and after treatment.Results:After treatment,the total effective rate in the observation group was significantly higher than in the control group( χ^(2)=3.998,P=0.046).The TCM syndrome scores of the observation group were significantly lower than those of the control group(P<0.05).The forced vital capacity(FVC)and forced expiratory volume in 1 second(FEV1)in the observation group were higher than those in the control group,while white blood cell(WBC)count,interleukin-6(IL-6),leukotriene B4(LT-B4),and high-sensitivity C-reactive protein(hs-CRP)levels were significantly lower than those in the control group(P<0.05).The SGRQ scores in the observation group were significantly lower than those in the control group(P<0.05).There were no significant adverse drug reactions in either group during the treatment period(P>0.05).Conclusion:Combining Xuanfei Huatan Sanyu Decoction with standard Western medical treatment for bronchiectasis with infection(phlegm-heat obstructing the lung syndrome)significantly improves lung function and immune capacity and effectively reduces inflammatory markers,offering an effective treatment option for this condition.
