化痰通络方联合依达拉奉右莰醇对缺血性脑卒中风痰阻络证患者血浆ICAM-1、脑钠肽表达的影响

摘　　要：目的:探讨化痰通络方联合依达拉奉右莰醇对缺血性脑卒中风痰阻络证患者血浆ICAM-1、脑钠肽表达的影响。方法:选取2020年3月—2023年3月江阴市中医院脑病科收治的缺血性脑卒中风痰阻络证患者80例,按随机数表法分为西药组与联合组,每组40例。西药组给予依达拉奉右莰醇治疗,联合组在西药组的基础上联合使用化痰通络方。治疗后比较两组患者临床疗效、中医证候积分、神经功能评分(NIHSS)、脑损伤标志物[细胞间黏附分子-1(ICAM-1)、脑钠肽]、炎症因子指标[白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、高敏C反应蛋白(hs-CRP)]及不良反应发生率的情况。结果:治疗后,两组患者的各项指标均较治疗前明显改善(P<0.01)。联合组的总有效率高于西药组(P<0.01)。联合组中医证候积分、NIHSS评分明显低于西药组(P<0.01),血浆ICAM-1和脑钠肽水平低于西药组(P<0.01),联合组的IL-2、IL-6及hs-CRP水平均明显低于西药组(P<0.01),两组患者的不良反应发生率比较差异无统计学意义(P>0.05)。结论:化痰通络方联合依达拉奉右莰醇治疗缺血性脑卒中风痰阻络证患者可显著改善临床疗效、中医证候及神经功能,并有效降低炎症水平,为临床治疗提供新的策略。Objective:To investigate the effect of Huatan Tongluo Formula combined with edaravone dexborneol on the expression of plasma ICAM-1 and brain natriuretic peptide(BNP)in patients with ischemic stroke of wind-phlegm blocking collaterals syndrome.Methods:A total of 80 patients with ischemic stroke of wind-phlegm blocking collaterals syndrome,admitted to the Department of Encephalopathy,Jiangyin Hospital of Traditional Chinese Medicine from March 2020 to March 2023,were selected and randomly divided into a Western medicine group and a combined group,with 40 patients in each group.The Western medicine group was treated with edaravone dexborneol,while the combined group received Huatan Tongluo Formula in addition to the treatment in the Western medicine group.After treatment,the clinical efficacy,Traditional Chinese Medicine(TCM)syndrome scores,neurological function scores(NIHSS),brain injury markers[intercellular adhesion molecule-1(ICAM-1),brain natriuretic peptide(BNP)],inflammatory factors[interleukin-2(IL-2),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP)],and adverse reaction rates were compared between the two groups.Results:After treatment,all indicators in both groups improved compared to before treatment(P<0.05).The overall effective rate in the combined group was higher than that in the Western medicine group(P<0.05).The TCM syndrome scores and NIHSS scores in the combined group were significantly lower than those in the Western medicine group(P<0.05).Plasma ICAM-1 and BNP levels were lower in the combined group than in the Western medicine group(P<0.05).Additionally,IL-2,IL-6,and hs-CRP levels were significantly lower in the combined group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Huatan Tongluo Formula combined with edaravone dexborneol significantly improves clinical efficacy,TCM syndromes,and neurological function in patients with ischemic stroke of wind-phlegm blocking collaterals syndrome,eff
