温阳化癥汤联合化疗治疗中晚期肝癌效果及对化疗不良反应的影响

摘　　要：目的:观察温阳化癥汤联合化疗治疗中晚期肝癌效果及对化疗不良反应影响。方法:选择2020年1月—2023年1月于海安市中医院就诊的中晚期肝癌患者89例,采用随机数字表法分成对照组(44例)与观察组(45例)。对照组患者行肝动脉化疗栓塞(TACE)治疗,观察组在对照组基础上加用温阳化癥汤口服治疗。观察两组患者临床疗效、中医证候积分、肿瘤标志物水平、不良反应与生存率分析。结果:观察组患者治疗客观有效率(ORR)与疾病控制率(DCR)均高于对照组,且两组DCR差异具有统计学意义(P<0.05);治疗后,观察组患者肝功能指标丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、白蛋白(ALB)、总胆红素(TBIL)高于对照组(P<0.01);治疗后,观察组中医证候积分、血清癌胚抗原(CEA)、甲胎蛋白(AFP)、E-钙黏蛋白(EC)水平、骨髓抑制发生率与不良反应总发生率均低于对照组(P<0.05,P<0.01);观察组无进展生存时间(PFS)长于对照组(Log Rank χ^(2)=19.002,P=0.000)。结论:温阳化癥汤联合化疗治疗中晚期肝癌疗效较好,可改善患者临床症状及肝功能水平,降低患者肿瘤标志物水平,并可使患者获得生存受益。To observe the effectiveness of Wenyang Huazheng Decoction combined with chemotherapy in the treatment of advanced liver cancer and its impact on chemotherapy-related adverse reactions.Methods:A total of 89 patients with advanced liver cancer who were treated at Hai an Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were selected and randomly divided into a control group(44 patients)and an observation group(45 patients)using a random number table method.The control group received transcatheter arterial chemoembolization(TACE)treatment,while the observation group was given Wenyang Huazheng Decoction in addition to the TACE treatment.Clinical efficacy,Traditional Chinese Medicine(TCM)symptom scores,tumor marker levels,adverse reactions,and survival rates were analyzed for both groups.Results:The objective response rate(ORR)and disease control rate(DCR)were higher in the observation group compared to the control group,with a statistically significant difference in DCR(P<0.05).After treatment,the liver function indicators,including alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB),and total bilirubin(TBIL),were higher in the observation group compared to the control group(P<0.05).The observation group also had lower TCM symptom scores,serum carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),E-cadherin(EC)levels,incidence of bone marrow suppression,and overall adverse reaction rates compared to the control group(P<0.05,P<0.01).The progression-free survival(PFS)of the observation group was longer than that of the control group(Log Rank χ^(2)=19.002,P=0.000).Conclusion:Wenyang Huazheng Decoction combined with chemotherapy is effective in treating advanced liver cancer,improving clinical symptoms and liver function,reducing tumor marker levels,and providing survival benefits to patients.
