自拟益肾平肝方结合耳穴推拿对青少年视疲劳肝肾阴虚证的疗效研究

摘　　要：目的:探讨自拟益肾平肝方结合耳穴推拿对青少年视疲劳肝肾阴虚证的疗效。方法:选择2019年1月—2022年12月于眉山市中医医院就诊的肝肾阴虚型青少年视疲劳患者122例,采用随机数字表法分为推拿组与联合组,每组61例。两组均给予七叶洋地黄双苷滴眼液治疗,在此基础上,推拿组给予耳穴推拿治疗,联合组给予自拟益肾平肝方结合耳穴推拿治疗。观察两组患者的中医证候积分、泪膜破裂时间(BUT)与泪液分泌试验(SIT)、眼血流动力学、临床疗效、不良反应发生情况。结果:治疗后,两组主症与次症的中医证候积分均低于治疗前(P<0.05),且联合组更低(P<0.05);两组BUT、SIT、睫状前动脉(ACA)、视网膜中央动脉(CRA)的收缩期峰值流速(PSV)和舒张末期血流速度(EDV)均高于治疗前(P<0.05),且联合组更高(P<0.05);联合组治疗总有效率高于推拿组(P<0.05);不良反应总发生率两组无明显差异(P>0.05)。结论:自拟益肾平肝方结合耳穴推拿可改善患者眼血流动力学,降低中医证候积分,延长患者泪膜破裂时间,提高临床疗效,且安全性较高。Objective:To investigate the efficacy of a self-formulated Yishen Pinggan Formula combined with auricular tuina in treating adolescent visual fatigue with liver and kidney yin deficiency syndrome.Methods:A total of 122 adolescents with liver and kidney yin deficiency-type visual fatigue who visited Meishan Hospital of Traditional Chinese Medicine from January 2019 to December 2022 were selected.They were randomly divided into an tuina group and a combined group,with 61 patients in each group.Both groups received treatment with Digitalis purpurea glycosides eye drops.Additionally,the tuina group received auricular tuina treatment,while the combined group received the self-formulated Yishen Pinggan Formula combined with auricular tuina treatment.The TCM syndrome scores,tear break-up time(BUT),tear secretion test(SIT),ocular hemodynamics,clinical efficacy,and incidence of adverse reactions were observed and compared between the two groups.Results:After treatment,the main and secondary TCM syndrome scores in the combined group were lower than those in the tuina group(P<0.05).The combined group showed higher BUT,SIT,and peak systolic velocity(PSV)and end-diastolic velocity(EDV)of the ciliary artery(ACA)and central retinal artery(CRA)compared to the tuina group(P<0.05).The total clinical efficacy rate in the combined group was higher than that in the tuina group(P<0.05).There was no significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Conclusion:The self-formulated Yishen Pinggan Formula combined with auricular tuina can improve ocular hemodynamics,reduce TCM syndrome scores,extend tear break-up time,and enhance clinical efficacy with relatively high safety.
