整叶法HoLEP治疗良性前列腺增生的效果及对性功能的影响

摘　　要：目的探讨整叶法前列腺钬激光剜除术(HoLEP)治疗良性前列腺增生(BPH)的效果及对性功能的影响。方法选择2018年7月至2022年7月在本院收治的68例BPH患者作为研究对象,按照治疗方案的差异将患者分为整叶组(31例)和对照组(37例)。整叶组开展整叶法HoLEP治疗,对照组开展前列腺电切术(TURP)治疗。分析两组患者的各项手术指标、手术前后性功能、术后并发症、术前后尿动力学等方面的差异。结果整叶组的尿管留置时间、住院时间均短于对照组,差异均有统计学意义(均P<0.05)。术后60 d,整叶组和对照组的IIEF-5评分分别为(11.05±2.06)分、(12.56±2.71)分,均低于术前[(14.52±3.10)分、(14.60±3.14)分],且整叶组低于对照组(均P<0.05)。整叶组的术后并发症总发生率低于对照组[12.90%(4/31)vs.43.24%(16/37),P<0.05]。整叶组术后的残余尿量(RVU)低于对照组,而最大尿流量(Q max)高于对照组,差异均有统计学意义(均P<0.05)。结论整叶法HoLEP治疗BPH的效果显著,可明显改善患者的性功能及尿动力学,且能降低并发症发生风险,值得临床推广应用。ObjectiveTo investigate the effect of holmium laser enucleation of the prostate(HoLEP)with en-bloc on benign prostatic hyperplasia(BPH)and its effect on sexual function.MethodsFrom July 2018 to July 2022,68 patients with BPH admitted to the hospital were selected,who were divided into whole leaf group(31 cases)and control group(37 cases)according to the difference of treatment plan.The whole leaf group received HoLEP with en-bloc,and the control group received TURP treatment.The differences of various surgical indexes,sexual function,postoperative complications,and urodynamics before and after surgery between the two groups were analyzed.ResultsThe indentation time and hospitalization time of the whole leaf group were shorter than those of the control group,and the differences were statistically significant(all P<0.05).The IIEF-5 scores of the whole leaf group and the control group were(11.05±2.06)points and(12.56±2.71)points at 60 days after surgery,respectively,which were lower than those before surgery(14.52±3.10)points and(14.60±3.14)points,and the whole leaf group was lower than the control group(all P<0.05).The total incidence of postoperative complications in the whole leaf group was lower than that in the control group[12.90%(4/31)vs.43.24%(16/37),P<0.05].The RVU of the whole leaf group was lower than that of the control group,and the maximum flow rate(Q max)was higher than that of the control group,with statistical significance(all P<0.05).ConclusionsHoLEP with en-bloc has a significant effect in the treatment of BPH,which can significantly improve the sexual function and urodynamics of patients,and reduce the risk of complications.It is worth popularizing and applying.
