摘  要:目的探究盆底超声参数联合血清弹性蛋白酶抑制剂(elafin)和转化生长因子-β1(TGF-β1)水平在预测产后压力性尿失禁中的应用价值。方法选取2021年1月至2023年1月本院收治的110例产后压力性尿失禁产妇为研究组,另选取同期未发生产后压力性尿失禁的110例产妇为对照组。采用盆底超声检查两组产妇的尿道旋转角(URA)、膀胱尿道后角(PUA)、膀胱颈移动度(BND);采用酶联免疫吸附法(ELISA)检测两组产妇的血清elafin和TGF-β1表达水平,比较两组产妇的URA、PUA、BND、elafin、TGF-β1。采用受试者工作特征(ROC)曲线分析盆底超声参数(URA、PUA、BND)联合血清elafin和TGF-β1对产后压力性尿失禁的预测价值。结果与对照组比较,研究组产妇的URA、PUA、BND以及血清elafin表达水平升高(均P<0.05),血清TGF-β1表达水平降低(P<0.05)。ROC曲线分析结果显示,URA、PUA、BND、elafin、TGF-β1单独预测产后压力性尿失禁的曲线下面积(AUC)分别为0.840、0.786、0.813、0.801和0.871,灵敏度分别为70.00%、70.91%、72.73%、69.09%和67.27%,特异度分别为85.45%、87.27%、83.64%、84.55%和89.09%;盆底超声参数和血清指标联合预测产后压力性尿失禁的AUC高于URA、PUA、BND、elafin、TGF-β1单独预测(均P<0.05)。结论盆底超声参数联合血清elafin、TGF-β1水平对产后压力性尿失禁的预测有较高的应用价值。ObjectiveTo explore the application value of pelvic floor ultrasound parameters combined with serum elafin and transforming growth factor-β1(TGF-β1)levels in predicting postpartum stress urinary incontinence.MethodsA total of 110 pregnant women with postpartum stress urinary incontinence admitted to our hospital from January 2021 to January 2023 were regarded as the study group,in addition,110 pregnant women who did not have post-natal stress urinary incontinence during the same period were regarded as the control group.Pelvic floor ultrasound was applied to examine the urethral rotation angle(URA),posterior vesicourethral angle(PUA),and bladder neck mobility(BND)of the two groups.The expression levels of serum elafin and TGF-β1 were detected by enzyme-linked immunosorbent assay(ELISA),and the URA,PUA,BND,elafin and TGF-β1 were compared between the two groups.The predictive value of pelvic floor ultrasound parameters(URA,PUA,BND)combined with serum elafin and TGF-β1 for postpartum stress urinary incontinence was analyzed by the receiver operating characteristic curve(ROC).ResultsCompared with the control group,the URA,PUA,BND and serum elafin expression level in the study group were obviously higher(all P<0.05).The serum TGF-β1 expression level was obviously lower(P<0.05).ROC results showed that the AUC of URA,PUA,BND,elafin and TGF-β1 predicted postpartum stress urinary incontinence was 0.840,0.786,0.813,0.801 and 0.871,respectively,the sensitivity was 70.00%,70.1%,72.73%,69.09%and 67.27%,and the specificity was 85.45%,87.27%,83.64%,84.55%and 89.09%,respectively.The AUC of the combined forecasting of pelvic floor ultrasound parameters and serum indicators for postpartum stress urinary incontinence was obviously higher than the AUC of URA,PUA,BND,elafin and TGF-β1(all P<0.05).ConclusionsPelvic floor ultrasound parameters combined with serum elafin and TGF-β1 levels have high application value in the prediction of postpartum stress urinary incontinence.
