摘 要:目的探讨妊娠期糖尿病(GDM)患者的血清视黄醇结合蛋白(RBP)、转铁蛋白(TRF)及尿微量白蛋白(mAlb)水平与肾损伤的相关性。方法选取2021年7月至2022年7月本院收治的98例GDM患者,根据有无肾损伤发生将患者分为GDM肾损伤组(31例)和GDM无肾损伤组(67例),另选同期产前检查的65例健康孕妇为对照组。分析入选对象的一般资料,检测三组研究对象的血清RBP、TRF、尿mAlb及肾功能指标[血清肌酐(Cr)、β2-微球蛋白(β2-MG)、尿酸(UA)]水平,采用Pearson相关性分析血清RBP、TRF、尿mAlb水平与肾功能的相关性,采用受试者工作特征(ROC)曲线分析血清RBP、TRF、尿mAlb单独及联合检测对肾损伤的预测效能。结果GDM肾损伤组、GDM无肾损伤组的血清RBP、Cr、β2-MG、UA及尿mAlb水平均高于对照组,血清TRF水平低于对照组(均P<0.05);且GDM肾损伤组的血清RBP、Cr、β2-MG、UA及尿mAlb水平高于GDM无肾损伤组,血清TRF水平低于GDM无肾损伤组(均P<0.05)。Pearson相关性分析结果显示,血清TRF与血清Cr、β2-MG、UA水平呈负相关(均P<0.05),血清RBP、尿mAlb与血清Cr、β2-MG、UA水平呈正相关(均P<0.05)。ROC曲线分析结果显示,血清RBP、TRF、尿mAlb及联合预测GDM患者肾损伤的曲线下面积(AUC)分别为0.900、0.888、0.912、0.976。结论血清RBP、TRF、尿mAlb水平与GDM患者肾损伤密切相关,三者联合检测的预测价值最高,有利于临床GDM的早期诊疗。ObjectiveTo investigate the correlation between serum retinol binding protein(RBP),transferrin(TRF),urinary microalbuminuria(mAlb)levels and kidney injury in gestational diabetes mellitus(GDM).MethodsFrom July 2021 to July 2022,98 patients with GDM in our hospital were selected,and were divided into GDM kidney injury group(31 cases)and GDM no kidney injury group(67 cases)according to the occurrence of kidney injury,and 45 healthy pregnant women undergoing prenatal examination during the same period were selected as control group.General data of the selected subjects were collected,and serum RBP,TRF,urinary mAlb and renal function indexes[serum creatinine(Cr),β2-microglobulin(β2-MG),uric acid(UA)]levels of the 3 groups were detected.Pearson correlation was used to analyze the correlation between serum RBP,TRF,urinary mAlb levels and renal function.ROC curve was used to analyze serum RBP,TRF,urinary mAlb and the predictive efficacy of combined detection of kidney injury.ResultsThe serum levels of RBP,Cr,β2-MG,UA and urinary mAlb in GDM group with kidney injury and GDM group without kidney injury were higher than those in control group,but the serum levels of TRF were lower than those in control group(all P<0.05).The serum levels of RBP,Cr,β2-MG,UA and urinary mAlb in GDM group with kidney injury were higher than those in GDM group without kidney injury(all P<0.05),and the serum level of TRF was lower than that in GDM group without kidney injury,the differences were statistically significant(all P<0.05).Pearson correlation analysis showed that serum TRF was negatively correlated with serum Cr,β2-MG and UA levels,while serum RBP and urine mAlb were positively correlated with serum Cr,β2-MG and UA levels(all P<0.05).ROC curve showed that serum RBP,TRF,urine mAlb and AUC of combined detection of kidney injury in GDM patients were 0.900,0.888,0.912 and 0.976.ConclusionsSerum RBP,TRF and urinary mAlb levels are closely related to kidney injury in GDM patients,and combined detection has the highest predictive value,
