和调督任安神针法长留针治疗中风后抑郁的临床观察

摘　　要：目的:观察和调督任安神针法长留针治疗中风后抑郁的临床疗效。方法:将70例中风后抑郁患者随机分为对照组和观察组,每组35例。两组患者除基础药物治疗外,对照组予以中风病常规针刺治疗和郁病常规针刺治疗,观察组予以中风病常规针刺治疗与和调督任安神针法长留针针刺治疗,均治疗4个疗程。观察治疗前、治疗4周及6个月随访时两组患者的汉密尔顿抑郁量表(HAMD)积分、抑郁自评量表(SDS)评分、匹兹堡睡眠质量指数量表(PSQI)评分、美国国立卫生研究院卒中量表(NIHSS)评分、日常生活活动能力量表(BI)评分,观察两组治疗方案的疗效。结果:治疗4周后,观察组总有效率(94.29%)高于对照组(77.14%),差异有统计学意义(P<0.05)。两组患者HAMD评分、SDS评分、PSQI评分、NIHSS评分均较治疗前降低,且观察组均低于对照组,差异有统计学意义(P<0.05)。两组患者BI指数评分较治疗前升高(P<0.05),且观察组高于对照组(P<0.05)。6个月后随访,两组患者HAMD评分、SDS评分、PSQI评分和NIHSS评分均较治疗4周时降低,差异有统计学意义(P<0.05)。两组患者BI指数评分较治疗4周时升高(P<0.05),且观察组高于对照组(P<0.05)。结论:和调督任安神针法长留针治疗中风后抑郁效果显著,可缓解抑郁状态,改善睡眠质量,有助于神经功能恢复和日常生活活动能力提升,疗效确切。Objective:To assess the clinical efficacy of regulating du-ren and tranquilizing mind acupuncture with long needle retention in the treatment of post-stroke depression(PSD).Methods:A total of 70 patients with PSD were randomly divided into a control group and an observation group,with 35 cases in each group.Both groups received basic medication,while the control group received conventional acupuncture for stroke and depression,and the observation group received conventional acupuncture for stroke along with long-stay acupuncture for regulating du-ren and tranquilizing mind.All patients underwent 4 treatment courses.The Hamilton Depression Scale(HAMD),self-rating depression scale(SDS),Pittsburgh Sleep Quality Index(PSQI),National Institutes of Health Stroke Scale(NIHSS),and activity of daily living scale(BI)scores were recorded before treatment,after 4 weeks of therapy,and at a 6-month follow-up.The clinical efficacy were compared between the two groups.Results:After 4 weeks of therapy,the total effective rate of the two groups was 94.29%,higher than that of the control group,which was 77.14%(P<0.05).The HAMD score,SDS score,PSQI score,and NIHSS score of both groups of patients decreased compared to before treatment,and the observation group was lower than the control group(P<0.05);The BI index scores of both groups of patients increased compared to before treatment(P<0.05),and the observation group was higher than the control group(P<0.05).After 6 months of follow-up,both groups of patients showed a decrease in HAMD score,SDS score,PSQI score,and NIHSS score compared to 4 weeks of treatment(P<0.05);The BI index scores of both groups of patients increased compared to 4 weeks of treatment(P<0.05),and the observation group was higher than the control group(P<0.05).Conclusion:The long-lasting needling method of regulating du-ren and tranquilizing mind acupuncture has a significant effect on post-stroke depression.It effectively alleviates depressive symptoms,improves sleep quality,promotes nerve function recovery,and
