摘　　要：基于张锡纯“大气下陷”学说,提出“大气陷则脑气筋损”理论,并据此探讨肌萎缩侧索硬化症(ALS)复杂病理机制。通过梳理文献发现中医脑气筋从起源部位、分布、数量及作用上来看与西医神经认识一致,并且脑气筋为大气所统摄,两者同充之于脾肾、同会之于溪谷,大气主管脑气筋司知觉运动之能。脑气筋司肌肉运动之能与运动神经元极为相似,均以中枢神经系统脑和脊髓传递运动冲动,从而支配肌肉自主运动,所以ALS发病责之脑气筋损。由于大气统摄脑气筋,脑气筋损为ALS发病之表面征象,其根本原因在于大气下陷。大气虚馁不得固守而下陷,为病发之起源,因虚致实,毒滞蚀髓为病进之关键,毒滞化热,热趁风势为病情急剧加重之驱动。Based on ZHANG Xichun's theory of"sinking of pectoral qi",this paper proposes the theory of"when the pectoral qi subsidenced,the brain and qi tendons are damaged"and discusses the complex pathological mechanisms of amyotrophic lateral sclerosis(ALS)accordingly.Through combing through the literature,we found that the brain qi tendons of Chinese medicine are consistent with the neurological understanding of Western medicine in terms of their origin,distribution,number and function,and that the brain qi tendons are controlled by the atmosphere,which is the same as that of the spleen and kidney,and the same as that of the Xigu,and that the qi is in charge of the cerebral qi tendons,which are responsible for the perceptual-motor function.Cerebral qi tendon division of muscle movement and motor neurons are very similar to the central nervous system brain and spinal cord to transmit motor impulses,thus dominating the voluntary muscle movement,so ALS onset of blame for brain qi tendon damage.As the atmosphere regulates the brain,the brain and the qi tendons,the brain and the qi tendons are damaged for the surface signs of ALS,and the root cause is the sinking of the atmosphere.The underlying cause of ALS is the sinking of the qi,which is the origin of the disease,and the toxic stagnation and erosion of the medulla oblongata is the key to the progression of the disease,and the stagnation of the toxin transforms into heat,and the heat takes advantage of the wind as the driving force for the rapid aggravation of the disease.
