醋酸亮丙瑞林联合孕三烯酮治疗子宫内膜异位症合并不孕症的临床效果

摘　　要：目的观察醋酸亮丙瑞林联合孕三烯酮治疗子宫内膜异位症合并不孕症的临床效果。方法回顾性分析2020年2月—2023年2月宁夏医科大学总医院生殖医学中心诊治的80例子宫内膜异位症合并不孕症患者临床资料,依据给药方案不同分为观察组42例和对照组38例。对照组单独服用孕三烯酮,观察组在对照组基础上加用醋酸亮丙瑞林,2组疗程均为6个月。比较2组临床疗效、生殖激素[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E 2)]水平、妊娠情况及不良反应。结果观察组总有效率为85.71%,高于对照组的65.79%(χ^(2)=4.373,P=0.037)。治疗6个月后,2组FSH、LH及E 2水平低于治疗前,且观察组低于对照组(P<0.01);随访1年,观察组妊娠率为47.62%,高于对照组的26.32%(χ^(2)=3.863,P=0.049);观察组与对照组不良反应总发生率比较差异无统计学意义(16.67%vs.13.16%,χ^(2)=0.193,P=0.661)。结论醋酸亮丙瑞林联合孕三烯酮治疗子宫内膜异位症合并不孕症有良好的疗效,可有效改善生殖激素水平,提高妊娠率,且不良反应发生率低,安全性高。Objective To observe the clinical effect of leuprorelin acetate combined with gestrinone in the treatment of endometriosis complicated with infertility.Methods The clinical data of 80 patients with endometriosis and infertility treated in Reproductive medicine center,General Hospital of Ningxia Medical University from February 2020 to February 2023 were retrospectively analyzed.According to the different dosing regimens,they were divided into observation group(42 cases)and control group(38 cases).The control group was treated with gestrinone alone,and the observation group was treated with leuprorelin acetate on the basis of the control group.The treatment course of both groups was 6 months.The clinical efficacy,reproductive hormone levels(FSH,LH,E 2),pregnancy and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 85.71%,which was higher than 65.79%of the control group(χ^(2)=4.373,P=0.037).After 6 months of treatment,the levels of FSH,LH and E 2 in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.01).Follow up for 1 year,the pregnancy rate in the observation group was 47.62%,which was higher than 26.32%in the control group(χ^(2)=3.863,P=0.049).There was no significant difference in the total incidence of adverse reactions between the observation group and the control group(16.67%vs.13.16%,χ^(2)=0.193,P=0.661).Conclusion Leuprorelin acetate combined with gestrinone in the treatment of endometriosis complicated with infertility has good curative effect,can effectively improve the level of reproductive hormone,improve the pregnancy rate,and has low incidence of adverse reactions and high safety.
