异甘草酸镁与甘草酸二铵分别联合舒肝宁治疗病毒性肝炎的临床效果比较

摘　　要：目的比较异甘草酸镁与甘草酸二铵分别联合舒肝宁治疗病毒性肝炎(VH)的临床效果。方法选取2022年7月—2023年12月扬州市第三人民医院收治的急慢性VH患者96例,以随机数字表法分为异甘草酸镁组与甘草酸二铵组,各48例。甘草酸二铵组采用甘草酸二铵联合舒肝宁注射液治疗,异甘草酸镁组采用异甘草酸镁联合舒肝宁注射液治疗,2组均持续2~4周。比较2组临床疗效,治疗前、治疗后4周肝功能指标[天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TBil)]、血清炎性因子[白介素(IL)-2、IL-10、肿瘤坏死因子-α(TNF-α)]及不良反应。结果异甘草酸镁组总有效率(93.75%)高于甘草酸二铵组(79.17%)(χ2=4.360,P=0.037)。与治疗前比较,治疗后4周,2组AST、ALT、TBil水平降低,且异甘草酸镁组低于甘草酸二铵组(P<0.01);2组IL-2水平升高,IL-10、TNF-α水平降低,且异甘草酸镁组变化幅度大于甘草酸二铵组(P<0.05或P<0.01)。异甘草酸镁组不良反应总发生率(6.25%)低于甘草酸二铵组(20.83%)(χ2=4.360,P=0.037)。结论异甘草酸镁联合舒肝宁治疗VH的效果更好,能有效降低患者转氨酶水平,减轻炎性反应,且用药安全性较高。Objective To compare the clinical effect of magnesium isoglycyrrhizinate and diammonium glycyrrhizinate combined with Shuganning in the treatment of viral hepatitis(VH).Methods A total of 96 patients with acute and chronic VH admitted to the Third People′s Hospital of Yangzhou from July 2022 to December 2023 were selected and randomly divided into magnesium isoglycyrrhizinate group and diammonium glycyrrhizinate group,48 cases in each group.The diammonium glycyrrhizinate group was treated with diammonium glycyrrhizinate combined with Shuganning injection,and the magnesium isoglycyrrhizinate group was treated with magnesium isoglycyrrhizinate combined with Shuganning injection.Both groups lasted for 2~4 weeks.The clinical efficacy,liver function indexes(AST,ALT,TBil),serum inflammatory factors(IL-2,IL-10,TNF-α)before and 4 weeks after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of magnesium isoglycyrrhizinate group(93.75%)was higher than that of diammonium glycyrrhizinate group(79.17%)(χ2=4.360,P=0.037).Compared with before treatment,the levels of AST,ALT and TBil in the two groups decreased 4 weeks after treatment,and magnesium isoglycyrrhizinate group was lower than diammonium glycyrrhizinate group(P<0.01).The levels of IL-2 increased,while the levels of IL-10 and TNF-αdecreased in the two groups,and the change amplitude of magnesium isoglycyrrhizinate group was greater than that of diammonium glycyrrhizinate group(P<0.05 or P<0.01).The total incidence of adverse reactions in magnesium isoglycyrrhizinate group(6.25%)was lower than that in diammonium glycyrrhizinate group(20.83%)(χ2=4.360,P=0.037).Conclusion The effect of magnesium isoglycyrrhizinate combined with Shuganning in the treatment of VH is better,which can effectively reduce the transaminase level of patients,reduce the inflammatory reaction,and has higher medication safety.
