宫炎平胶囊联合银尔舒凝胶治疗霉菌性阴道炎伴慢性宫颈炎的临床效果及对炎性因子的影响

摘　　要：目的观察宫炎平胶囊联合银尔舒凝胶治疗霉菌性阴道炎(FV)伴慢性宫颈炎的临床效果及对炎性因子的影响。方法选取2023年1月—2024年1月厦门大学附属第一医院妇产科收治的FV伴慢性宫颈炎患者100例,以随机数字表法分为宫炎平联合组和银尔舒组各50例。2组均应用常规基础治疗,此外银尔舒组给予银尔舒凝胶阴道给药治疗,宫炎平联合组在银尔舒组基础上给予宫炎平胶囊治疗,2组均治疗4周。比较2组疗效、症状消失时间,治疗前后免疫指标(CD4+、CD8+、CD4+/CD8+)、阴道分泌物炎性指标[白介素(IL)-2、IL-4、IL-10、干扰素-γ(IFN-γ)、细胞因子比值(Th1/Th2)],以及不良反应。结果宫炎平联合组总有效率为96.00%,高于银尔舒组的80.00%(χ2=6.061,P=0.014);宫炎平联合组阴道瘙痒、阴道灼热感及白带异常消失时间均短于银尔舒组(P<0.01)。治疗4周后,2组CD4+、CD4+/CD8+升高,CD8+降低,且宫炎平联合组高/低于银尔舒组(P<0.01);2组阴道分泌物IL-4、IL-10水平均下降,IL-2、IFN-γ、IL-2/IL-4、IL-2/IL-10、IFN-γ/IL-4、IFN-γ/IL-10均升高,且宫炎平联合组低/高于银尔舒组(P<0.01)。宫炎平联合组与银尔舒组不良反应总发生率比较差异不显著(10.00%vs.8.00%,χ2=0.122,P=0.727)。结论宫炎平胶囊联合银尔舒凝胶治疗FV伴慢性宫颈炎效果肯定,有助于促进阴道炎症的控制,调节Th1/Th2细胞因子平衡,增强免疫力,且不良反应少,安全性高。Objective To observe the clinical effect of Gongyanping capsule combined with Yinershu gel on fungal vaginitis(FV)with chronic cervicitis and its effect on inflammatory factors.Methods A total of 100 patients with FV with chronic cervicitis admitted to the Department of Obstetrics and Gynecology of the First Affiliated Hospital of Xiamen University from January 2023 to January 2024 were selected and divided into Gongyanping combined group and Yinershu group with 50 cases in each group by random number table method.Yinershu group was given Yinershu gel for vaginal medication,Gongyanping combined group was given Gongyanping capsule on the basis of Yinershu group,and both groups were treated for 4 weeks.The efficacy,symptom resolution time,immune indexes(CD4+,CD8+,CD4+/CD8+),inflammatory indexes of vaginal secretion(IL-2,IL-4,IL-10,IFN-γ,Th1/Th2)before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of Gongyanping group was 96.00%,which was higher than 80.00%of Yinershu group(χ2=6.061,P=0.014).The resolution time of vaginal pruritus,vaginal burning sensation and abnormal leucorrhea in Gongyanping combined group were shorter than those in Yinershu group(P<0.01).After 4 weeks of treatment,CD4+,CD4+/CD8+increased and CD8+decreased in two groups,and Gongyanping combined group were higher/lower than Yinershu group(P<0.01).The levels of IL-4,IL-10 in vaginal secretions of two groups were decreased,while the levels of IL-2,IFN-γ,IL-2/IL-4,IL-2/IL-10,IFN-γ/IL-4 and IFN-γ/IL-10 were increased,and the levels of Gongyanping combined group were lower/higher than those of Yinershu group(P<0.01).There was no significant difference in the overall incidence of adverse reactions between Gongyanping combined group and Yinershu group(10.00%vs.8.00%,χ2=0.122,P=0.727).Conclusion Gongyanping capsule combined with Yinershu gel has a positive effect in the treatment of FV with chronic cervicitis,which helps to promote the control of vaginal inflammation,regulate the bala
