SHP2和VEGF-A在卵巢子宫内膜异位症中的表达及临床意义

摘　　要：目的分析Src同源2结构域的蛋白酪氨酸磷酸酶(SHP2)与血管内皮生长因子A(VEGF-A)在卵巢子宫内膜异位症(OEM)中的表达及临床意义。方法收集53例经病理学证实为OEM患者的异位和在位内膜组织及33例正常子宫内膜组织。采用RT-qPCR法和免疫组织化学法检测各组内膜组织中SHP2、VEGF-A的表达,并分析其与疾病分期、痛经程度的关系。结果OEM患者异位内膜组织中SHP2和VEGF-A mRNA和蛋白水平均明显高于在位内膜和正常内膜组织(P<0.05)。R-AFS分期Ⅲ~Ⅳ期的OEM患者SHP2、VEGF-A mRNA和蛋白水平均高于Ⅰ~Ⅱ期患者(P<0.05)。重度痛经OEM患者的SHP2、VEGF-A mRNA和蛋白水平均高于轻度痛经者(P<0.05)。Spearman相关性分析结果显示,OEM患者异位组织中SHP2和VEGF-A mRNA水平呈正相关(r=0.369,P=0.007),SHP2和VEGF-A蛋白表达水平呈显著正相关(r=0.775,P=0.000)。结论SHP2、VEGF-A在OEM患者异位内膜组织中异常高表达,且与OEM的发生发展相关;但二者表达均不受月经周期及性激素调节的影响。Objective To analyze the expression and clinical significance of src homology 2 domain containing protein tyrosine phosphatase(SHP2)and vascular endothelial growth factor-A(VEGF-A)in ovarian endometriosis(OEM).Methods The ectopic and in-place endometrium tissues of 53 cases confirmed by pathology as OEM patients and 33 normal endometrium tissues were selected.RT-qPCR and immunohistochemistry were used to detect the expression of SHP2 and VEGF-A in endometrial tissues of each group,and their relationship with disease stage and dysmenorrhea degree was analyzed.Results The mRNA and protein levels of SHP2 and VEGF-A in ectopic endometrium tissues of the OEM patients were significantly higher than those in in-place and normal endometrium tissues(P<0.05).The mRNA and protein levels of SHP2 and VEGF-A in OEM patients with R-AFS stageⅢ‒Ⅳwere higher than those in patients with R-AFS stageⅠ‒Ⅱ(P<0.05).The mRNA and protein levels of SHP2 and VEGF-A in OEM patients with severe dysmenorrhea were higher than those in patients with mild dysmenorrhea(P<0.05).Spearman correlation analysis showed that the mRNA expression levels of SHP2 and VEGF-A in ectopic tissues of OEM patients were positively correlated(r=0.369,P=0.007),and the protein expression levels of SHP2 and VEGF-A were significantly positively correlated(r=0.775,P=0.000).Conclusion The expression of SHP2 and VEGF-A in the ectopic endometrial tissue of OEM patients were abnormally high and were correlated with the occurrence and development of OEM.However,the expressions of SHP2 and VEGF-A in ovarian endometriosis were not affected by the regulation of menstrual cycle and sex hormones.
