摘  要:目的 探讨呼出气一氧化氮(Fe NO)水平、外周血嗜酸性粒细胞(EOS)计数对儿童支气管哮喘控制水平的评估价值,以期为临床相关治疗提供参考。方法 选取2022年10月—2024年1月于民航总医院儿童呼吸过敏门诊诊治的160例支气管哮喘患儿作为疾病组,并选取80例同期健康体检儿童为对照组。疾病组患儿根据入院时儿童哮喘控制测试问卷(C-ACT)评分分组,评分≥20分的支气管哮喘患儿分入控制组,评分<20分的支气管哮喘患儿则分入未控制组。收集160例支气管哮喘患儿年龄、性别、身高、身体质量指数(BMI)、家庭成员结构、家长吸烟情况、家长文化程度、哮喘家族史、过敏体质情况以及Fe NO水平、外周血EOS计数;同时采集对照组研究对象Fe NO水平、外周血EOS计数信息。结果 疾病组患儿Fe NO水平、外周血EOS计数高于对照组研究对象,差异均有统计学意义(P<0.05)。未控制组患儿Fe NO水平、外周血EOS计数高于控制组,差异均有统计学意义(P<0.05)。多因素logistic回归分析结果显示:过敏体质(OR=2.115;95%CI:1.296~3.452),Fe NO≥42.67 ppb(OR=3.770;95%CI:1.804~7.877),外周血EOS计数≥0.77×10~9/L(OR=2.875;95%CI:1.560~5.299)是影响支气管哮喘患儿哮喘控制水平的危险因素(P<0.05)。结论 Fe NO水平和外周血EOS计数的上升与支气管哮喘患儿哮喘控制水平有一定联系,可作为评估支气管哮喘患儿哮喘控制水平的生物标记物。Objective To explore the value of fractional exhaled nitric oxide(FeNO)levels and peripheral blood eosinophil(EOS)counts in assessing the control level of bronchial asthma in children,aiming to provide a reference for clinical treatment.Methods A total of 160 children with bronchial asthma treated in the pediatric respiratory allergy clinic of Civil Aviation General Hospital from October 2022 to January 2024 were selected as the disease group,and 80 healthy children who underwent physical examination during the same period were selected as the control group.Children in the disease group were grouped based on the Children's Asthma Control Test(C-ACT)questionnaire score at admission,with those scoring≥20 points placed in the controlled group and those scoring<20 points in the uncontrolled group.Data on age,gender,height,BMI,family structure,parental smoking status,highest parental education level,family history of asthma,allergic constitution,and FeNO levels and peripheral blood EOS counts of 160 children with bronchial asthma were collected.Simultaneously,FeNO levels and peripheral blood EOS counts of the control group participants were also collected.Results The Fe-NO levels and peripheral blood EOS counts in children of the disease group were significantly higher than those in the control group participants(P<0.05).The FeNO levels and peripheral blood EOS counts in the uncontrolled group were significantly higher than those in the controlled group(P<0.05).Multifactorial logistic regression analysis results showed that allergic constitution(OR=2.115;95%CI:1.296-3.452),FeNO≥42.67 ppb(OR=3.770;95%CI:1.804-7.877),and peripheral blood EOS count≥0.77×109/L(OR=2.875;95%CI:1.560-5.299)are risk factors affecting the asthma control level in children with bronchial asthma(P<0.05).Conclusion The increase in FeNO levels and peripheral blood EOS counts is correlated with the asthma control level in children with bronchial asthma and can be used as a biomarker for assessing the asthma control level in children with bron
