摘 要:目的比较微等离子体射频、CO_(2)点阵激光治疗体表增生性瘢痕的效果,探讨其安全性。方法2021年1月—2023年5月河南省人民医院诊治体表增生性瘢痕患者64例,32例自愿选择CO_(2)点阵激光治疗者为CO_(2)点阵激光组,32例自愿选择微等离子体射频治疗者为微等离子体组。2组患者均治疗3次,每次治疗间隔2个月。治疗6个月时评估疗效,计算治疗前与治疗6个月时温哥华瘢痕标度法评分的差值反映瘢痕改善程度;采用治疗满意度评估疗效,患者基本满意、满意为有效,不满意为无效;记录2组治疗疼痛及治疗期间色素沉着、持续红斑等并发症发生情况,比较并发症发生率。结果治疗6个月时,微等离子体组温哥华瘢痕标度法评分差值[(1.72±0.81)分]大于CO_(2)点阵激光组[(1.31±0.64)分](t=2.216,P=0.030),治疗有效率(96.9%)高于CO_(2)点阵激光组(87.5%)(χ^(2)=5.143,P=0.023)。CO_(2)点阵激光组发生治疗疼痛5例,色素沉着4例,持续红斑5例;微等离子体组发生治疗疼痛8例,色素沉着3例,持续红斑2例;微等离子体组并发症发生率(40.63%)与CO_(2)点阵激光组(43.75%)比较差异无统计学意义(χ^(2)=0.064,P=0.800)。结论微等离子体射频治疗体表增生性瘢痕的疗效优于CO_(2)点阵激光,且不增加不良反应。Objective To compare the efficacy of micro-plasma radiofrequency and CO_(2)fractional laser in the treatment of hypertrophic scar on the body surface,and to explore its safety.Methods From January 2021 to May 2023,64 patients with hypertrophic scar were treated in Henan Provincial People's Hospital,among whom 32 patients voluntarily chose CO_(2)fractional laser treatment(CO_(2)fractional laser group)and the other 32 patients voluntarily chose micro-plasma radiofrequency treatment(micro-plasma group).The patients in both groups were treated 3 times,with an interval of2 months.The efficacy was evaluated after 6-month treatment.The difference value of the Vancouver Scar Scale before and after 6-month treatment was calculated to reflect the degree of scar improvement.The efficacy was assessed by patient treatment satisfaction,with basically satisfied and satisfied indicating effective and dissatisfied indicating ineffective.Complications as treatment pain,and pigmentation and persistent erythema during treatment were recorded in two groups,and its incidence rate was compared between two groups.Results After 6-month treatment,the difference value of Vancouver Scar Scale was greater in micro-plasma group(1.72±0.81)than that in CO_(2)fractional laser group(1.31±0.64)(t=2.216,P=0.030).The effective rate of treatment was higher in micro-plasma group(96.9%)than that in CO_(2)fractional laser group(87.5%)(χ^(2)=5.143,P=0.023).There were 5 cases of treatment pain,4 cases of pigmentation,and 5 cases of persistent erythema in CO_(2)fractional laser group,and 8 cases of treatment pain,3 cases of pigmentation,and 2 cases of persistent erythema in micro-plasma group.There was no significant difference in the rate of complications between micro-plasma group(40.63%)and CO_(2)fractional laser group(43.75%)(χ^(2)=0.064,P=0.800).Conclusion Micro-plasma radiofrequency is more effective than CO_(2)fractional laser in the treatment of hypertrophic scar on the body surface and does not increase the adverse reactions.
