摘  要:哮喘伴不可逆或固定气流阻塞是哮喘的一种严重临床表型,目前尚缺乏有效的预防、治疗方法。新的证据表明,哮喘的固定气流阻塞不仅限于气道改变,肺实质同样也受到了影响。环境暴露与哮喘固定气流阻塞密切相关,室内外环境中的污染物和刺激物可导致气道和肺组织的慢性炎症,加重气道和肺实质的损伤,促进气道、肺实质重塑,进而引起固定气流阻塞。本文就哮喘气道重塑、肺实质重塑的特征及对固定气流阻塞的影响,环境暴露在哮喘固定气流阻塞中作用的研究进展作一综述。Asthma with irreversible or fixed airflow obstruction is a severe clinical phenotype of asthma,and there is still a lack of effective prevention and treatment methods at present.New evidence shows that the fixed airflow obstruction in asthma not only changes the airway,but also affects the lung parenchyma.Environmental exposure is closely corelated with the fixed airflow obstruction in asthma.Pollutants and irritants in indoor and outdoor environments would cause chronic inflammation of the airway and lung tissue,aggravate the damage to the airway and lung parenchyma,promote airway and lung parenchyma remodeling,and lead to fixed airflow obstruction.This paper reviews the research progress of the characteristics of airway and lung parenchyma remodeling in asthma and their impacts on fixed airflow obstruction,as well as the role of environmental exposure in fixed airflow obstruction in asthma.
