摘 要:目的 探讨高校教职工高同型半胱氨酸(HHcy)和高血清尿酸(HUA)对早期肾功能损伤的交互作用,为该部分人群有效早期干预及预防保健工作提供数据参考。方法 收集某高校体检数据库中2020年参加健康体检教职工的体检结果数据,分析3 273名教职工空腹静脉血中同型半胱氨酸(Hcy)、血清尿酸(UA)及血肌酐等体检结果数据,采用logistic回归模型和限制性立方样条(RCS)曲线分析HHcy、HUA与早期肾功能损伤的关系,运用相加模型评估二者交互作用对不同性别、年龄教职工早期肾功能损伤的影响。结果 纳入教职工年龄为(48.91±13.98)岁,其中男性1 522名(46.5%),早期肾功能损伤患者430名(13.14%)。HHcy (OR=2.20,95%CI:1.67~2.90)、HUA (OR=3.55,95%CI:2.62~4.81)患者早期肾功能损伤患病率均升高(P<0.01)。仅在男性或60岁以内教职工中发现HHcy和HUA对早期肾功能损伤患病风险存在协同作用,其患病效应分别是二者单独作用之和的2.60倍(SI=2.6,95%CI:1.24~5.47)和4.00倍(SI=4,95%CI:1.68~9.25);由两因素协同作用所致患病率分别为其他因素所致患病率的4.65倍(RERI=4.65,95%CI:0.44~8.87)和7.90倍(RERI=7.9,95%CI:1.32~14.49);分别有61.51%和74.95%的患病风险由血UA升高和Hcy升高的协同作用所致。结论 HHcy和HUA分别与早期肾功能损伤存在正向关联;HHcy和HUA对男性或60岁以内教职工早期肾功能损伤存在交互作用。Objective To explore the interaction between hyperhomocysteinemia and hyperuricemia on risk of early renal function injury among college staff of different gender and age. Methods The test results of serum homocysteine, serum uric acid and serum creatinine of 3 273 faculty members who participated in physical examination in a university in 2020 were analyzed.Logistic regression model and restricted cubic spline(RCS) curve were used to analyze the relationship between HHcy, HUA and early renal function injury.The additive model was used to evaluate their interaction on risk of early renal function injury among college staff of different sex and age. Results The average age of the subjects was(48.91± 13.98) years old, 1 522 were males(46.5%) and 430(13.14%) had early renal function decline.Compared with the non-HHcy and non-HUA college staff, the prevalence of early renal function injury in college staff with HHcy(OR=2.20,95%CI:1.67-2.90)and HUA(OR=3.55,95%CI:2.62-4.81)increased respectively(P<0.01).Only in males or those younger than 60 years old, there was a synergistic effect between HHcy and HUA on the risk of early renal function injury.When the 2 factors act together, the synergistic effect was 2.60(SI=2.6,95%CI:1.24-5.47) and 4.00(SI=4,95%CI:1.68-9.25) times of the sum of the 2 effects, The morbidity rates caused by the 2 factors were 4.65(RERI= 4.65,95%CI:0.44-8.87) and 7.90(RERI=7.9,95%CI:1.32-14.49) times higher than those caused by other factors, respectively.In all subjects with early renal function injury, 61.51% and 74.95% of the risk were caused by the synergistic effect of the 2 factors, respectively. Conclusion HHcy and HUA were positively correlated with early renal function injury, respectively.HHcy and HUA have interaction on early renal function damage among males and college staff younger than 60 years old.
