摘 要:目的:探究在产前胎儿畸形筛查中运用超声诊断的作用价值。方法:回顾性分析2022年4月至2024年4月期间在我院足月生产的68例产妇产前超声资料(均为分娩后或引产后畸形),均接受二维、三维、四维超声检查。比较单一检测与联合检测的胎儿畸形诊断结果。结果:68例研究对象的引产或分娩结果得出,共有95处胎儿畸形情况。联合检查检出率100.00%(95/95)、四维超声检出率98.95%(94/95)分别与引产或分娩结果比较,均无统计学差异(t=0.000,P=1.000;t=1.005,P=0.316);而二维超声检出率91.58%(87/95)、三维超声检出率95.79%(91/95)均低于联合检查与引产或分娩结果,有统计学差异(t=8.352,P=0.004;t=4.086,P=0.043)。二维超声、三维超声的胎儿畸形诊断结果(85.29%、94.12%)均低于联合检查(100.00%),且二维超声诊断结果低于四维超声结果(98.53%),数据比对有统计学差异(P<0.05)。而四维超声诊断结果与联合检查、三维超声与四维超声诊断结果、二维超声与三维超声诊断结果的数据比较均不具有统计学差异(P>0.05)。结论:在产前胎儿畸形筛查中采用联合超声诊断,可提高产前胎儿畸形检出率,减少漏诊率,提高产前胎儿畸形超声诊断筛查价值。Objective:To explore the value of ultrasound diagnosis in prenatal fetal malformation screening.Methods:The prenatal ultrasound data of 68 full-term parturients in our hospital from April,2022 to April,2024 were retrospectively analyzed,all of them were examined by two-dimensional,threedimensional and four-dimensional ultrasound.To compare the diagnosis results of fetal malformation between single detection and combined detection.Results:The results of induced labor or delivery of 68 subjects showed that there were 95 fetal malformations.The detection rate of combined examination was 100.00%(95/95),and the detection rate of four-dimensional ultrasound was 98.95%(94/95),and there was no statistical difference between them(t=0.000,P=1.000;t=1.005,P=0.316).The detection rate of two-dimensional ultrasound was 91.58%(87/95),and the detection rate of three-dimensional ultrasound was 95.79%(91/95),both of which were lower than the results of combined examination and induced labor or delivery,with statistical differences(t=8.352,P=0.004;t=4.086,P=0.043).The diagnosis results of fetal malformation by two-dimensional ultrasound and three-dimensional ultrasound(85.29%,94.12%)were lower than those by combined examination(100.00%),and the diagnostic results of two-dimensional ultrasound were lower than those of four-dimensional ultrasound(98.53%),and the difference between the diagnosis results of two-dimensional ultrasound and other methods was statistically significant(P<0.05).However,there was no statistically significant difference(P>0.05)between the results of fourdimensional ultrasound diagnosis and joint examination,three-dimensional ultrasound and fourdimensional ultrasound diagnosis,and two-dimensional ultrasound and three-dimensional ultrasound diagnosis.Conclusion:Using combined ultrasound diagnosis in prenatal fetal malformation screening can improve the detection rate of prenatal fetal abnormalities,reduce the missed diagnosis rate and improve the screening value of prenatal fetal malformation ultrasound diagnosi
