支气管哮喘患儿外周血IgG4 TIM4 MMP-12及嗜酸性粒细胞的表达特点与意义

摘　　要：目的 探讨支气管哮喘患儿外周血免疫球蛋白G4(IgG4)、T细胞免疫球蛋白域粘蛋白域蛋白4(TIM4)、基质金属蛋白酶-12(MMP-12)、嗜酸性粒细胞的表达特点及意义。方法 选取2021年5月—2022年12月金华市妇幼保健院收治的支气管哮喘患儿82例,根据支气管哮喘发作情况分为缓解期组28例,急性发作期组54例。另收集在金华市妇幼保健院体检的健康儿童25例为健康对照组。检测3组血清IgG4、TIM4、MMP-12、嗜酸性粒细胞水平。急性发作期支气管哮喘患儿按病情严重程度分为轻度组、中度组、重度组。随访1年,患儿填写哮喘控制测试评分表(ACTC-ACT),分为预后不良组和预后良好组。结果 3组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平比较差异均有统计学意义(均P<0.05)。急性发作期组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平[(0.95±0.13)g/L、(1.18±0.15)ng/ml、(25.42±4.41)pg/ml、(0.57±0.09)×10^(9)/L]均明显高于缓解期组[(0.62±0.16)g/L、(0.74±0.19)ng/ml、(21.93±3.75)pg/ml、(0.35±0.06)×10^(9)/L]和健康对照组,缓解期组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平均明显高于健康对照组(均P<0.05)。轻度组、中度组、重度组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平比较差异均有统计学意义(均P<0.05)。重度组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平[(1.48±0.12)g/L、(1.72±0.17)ng/ml、(29.29±4.85)pg/ml、(0.83±0.10)×10^(9)/L]均明显高于中度组[(0.96±0.11)g/L、(1.19±0.14)ng/ml、(25.81±4.26)pg/ml、(0.60±0.08)×10^(9)/L]和轻度组,中度组外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平均明显高于轻度组(均P<0.05)。预后不良组患儿外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞水平均明显高于预后良好组患儿(均P<0.05)。受试者工作特征(ROC)曲线结果显示:外周血IgG4、TIM4、MMP-12、嗜酸性粒细胞对急性发作期支气管哮喘患儿预后均具有较高的预测价Objective To explore the expression characteristics and significance of peripheral blood immunoglobulin G4(IgG4),T cell immunoglobulin domain mucin domain protein 4(TIM4),matrix metalloproteinase 12(MMP12),and eosinophils in children with bronchial asthma.Methods A total of 82 children with bronchial asthma admitted to Jinhua Maternal and Child Health Hospital from May 2021 to December 2022 were selected.According to the severity of bronchial asthma attacks,they were divided into remission group of 28 ca-ses and acute attack group of 54 cases.A total of 25 healthy children who underwent physical examination in Jinhua Maternal and Child Health Hospital were collected as healthy control group.The levels of serum IgG4,TIM4,MMP12,and eosinophils in the three groups were detected.The children with acute exacerbation of bronchial asthma were divided into mild group,moderate group,and severe group accord-ing to the severity of the disease.After follow-up for 1 year,the patients filled out Asthma Control Test Score(ACTC-ACT)and were divid-ed into poor prognosis group and good prognosis group.Results There were statistically significant differences in the levels of IgG4,TIM4,MMP12,and eosinophils in the peripheral blood among the three groups(all P<0.05).The levels of IgG4,TIM4,MMP12,and eosino-phils in peripheral blood of acute attack group[(0.95±0.13)g/L,(1.18±0.15)ng/ml,(25.42±4.41)pg/ml,(0.57±0.09)xl09/L]were significantly higher than those in remission group and healthy control group.The levels of IgG4,TIM4,MMP12,and eosinophils in pe-ripheral blood of remission group were significantly higher than those in healthy control group(all P<0.05).There were statistically signifi-cant differences in the levels of IgG4,TIM4,MMP12,and eosinophils in peripheral blood among mild,moderate,and severe groups(all P<0.05).The levels of IgG4,TIM4,MMP12,and eosinophils in peripheral blood of severe group[(1.48±0.12)g/L,(1.72±0.17)ng/ml,(29.29±4.85)pg/ml,(0.83±0.10)×10^(9)/L]were significantly higher than those in moderate gro
