散寒活血祛白方联合药线点灸治疗白癜风的疗效观察及皮损复色模式分析

摘　　要：目的探讨散寒活血祛白方联合药线点灸治疗白癜风的临床疗效,以及对皮损复色模式的影响。方法选取医院皮肤科2022年5月至2023年12月收治的成人白癜风患者130例,按随机数字表法分为联合组和对照组,各65例。两组患者均予散寒活血祛白方内服,同时外用散寒活血祛白酊,联合组加用药线点灸治疗,均连续治疗12周。结果联合组的总有效率为74.60%,明显高于对照组的52.38%(P<0.05)。治疗后,两组患者的白癜风面积评分指数(VASI)、皮肤病生活质量指数调查问卷(DLQI)评分和中医证候积分均明显降低(P<0.05),且联合组均明显低于对照组(P<0.05);两组患者的调节性T淋巴细胞(Treg)占CD4+T淋巴细胞百分比明显升高(P<0.05),Th17细胞占CD4+T淋巴细胞百分比及白细胞介素17、转化生长因子-β水平均明显降低(P<0.05),且联合组均明显优于对照组(P<0.05)。联合组和对照组边缘型、点状型、均一型、混合型复色模式占比相当(37.10%比44.17%,19.35%比18.33%,14.52%比12.50%,29.03%比25.00%,P>0.05),不良反应发生率相当(3.17%比4.76%,P>0.05)。结论散寒活血祛白方联合药线点灸治疗白癜风的临床疗效良好,能改善患者的皮肤白斑,减轻中医症状,提高生活质量,且安全性良好。Objective To investigate the clinical efficacy of Sanhan Huoxue Qubai Recipe combined with medicinal line moxibustion in the treatment of vitiligo,and its effect on the skin repigmentation patterns of skin lesions.Methods A total of 130 adult patients with vitiligo admitted to the hospital from May 2022 to December 2023 were selected and divided into the combined group and the control group by the random number table method,with 65 cases in each group.The patients in the two groups were given the oral administration of Sanhan Huoxue Qubai Recipe and external application of Sanhan Huoxue Qubai Tincture,on this basis,the patients in the combined group were treated with medicinal line moxibustion.Both groups were treated for 12 weeks.Results The total effective rate in the combined group was 74.60%,which was significantly higher than 52.38%in the control group(P<0.05).After treatment,the vitiligo area score index(VASI),dermatology quality of life index questionnaire(DLQI)score,and traditional Chinese medicine(TCM)syndrome scores in the two groups significantly decreased(P<0.05),and those in the combined group were significantly lower than those in the control group(P<0.05);the percentage of regulatory T lymphocytes(Treg)to CD4+T lymphocytes in the two groups significantly increased(P<0.05),while the percentage of Th17 cells to CD4+T lymphocytes and the levels of interleukin-17 and transforming growth factor-βin the two groups significantly decreased(P<0.05),and those in the combined group were significantly better than those in the control group(P<0.05).The proportion of edge type,dot type,homogeneous type,and mixed color patterns in the combined group and the control group was comparable(37.10%vs.44.17%,19.35%vs.18.33%,14.52%vs.12.50%,29.03%vs.25.00%,P>0.05),and the incidence of adverse reactions in the combined group and the control group was comparable(3.17%vs.4.76%,P>0.05).Conclusion Sanhan Huoxue Qubai Recipe combined with medicinal line moxibustion has good clinical efficacy and safety in the treatment of vit
