关节镜下清理术联合加味身痛逐瘀汤对膝关节急性创伤性滑膜炎患者关节功能的影响

摘　　要：目的:探讨关节镜下清理术联合加味身痛逐瘀汤对膝关节急性创伤性滑膜炎患者关节功能的影响。方法:选择2020年6月—2023年5月徐州市中医院收治96例的膝关节急性创伤性滑膜炎患者,随机均分为对照组和观察组,各48例。对照组均进行关节镜下清理术,观察组在对照组基础上联合加味身痛逐瘀汤治疗。对比治疗前后两组中医症候积分、临床疗效、视觉模拟评分法(VAS)评分、Lysholm膝关节评分、西安大略和麦克马斯特大学关节炎指数(WOMAC)评分、炎症指标和不良反应发生情况。结果:治疗后,观察组主症、次症评分及总分均低于对照组,差异均有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组VAS评分、WOMAC评分均低于对照组,Lysholm膝关节评分高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组肿瘤坏死因子-α(TNF-α)、人基质金属蛋白酶-3(MMP-3)和白细胞介素-6(IL-6)水平均低于对照组,差异均有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:关节镜下清理术联合加味身痛逐瘀汤治疗膝关节急性创伤性滑膜炎患者的效果显著,更利于患者膝关节功能的恢复,缓解疼痛,增强疗效,且不增加不良反应。Objective:To investigate the effect of arthroscopic debridement combined with Modified Shentong Zhuyu Decoction on joint function in patients with acute traumatic synovitis of knee joint.Method:A total of 96 patients with acute traumatic synovitis of knee joint admitted to Xuzhou Hospital of Traditional Chinese Medicine from June 2020 to May 2023 were randomly divided into control group and observation group,with 48 cases in each group.The control group underwent arthroscopic debridement,and the observation group was treated with Modified Shentong Zhuyu Decoction on the basis of the control group.Traditional Chinese medicine syndrome score,clinical efficacy,visual analogue scale(VAS)score,Lysholm knee joint score,Western Ontario and McMaster University osteoarthritis index(WOMAC)score,inflammation index before and after treatment and adverse reactions between two groups were compared.Result:After treatment,the main symptoms,secondary symptoms scores and total scores of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,VAS score and WOMAC score of observation group were lower than those in control group,Lysholm knee score was higher than that in control group,the differences were statistically significant(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),human matrix metalloproteinase-3(MMP-3)and interleukin-6(IL-6)in observation group were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion:Arthroscopic debridement combined with Modified Shentong Zhuyu Decoction has a significant effect in the treatment of acute traumatic synovitis of knee joint,which is more conducive to the recovery of knee joint function,reliev
