摘　　要：目的:探讨接触-介绍-沟通-询问-回答-离开(CICARE)沟通模式在儿童扁桃体腺样体术前访视中的应用效果。方法:选取2021年6月—2023年6月江西省儿童医院收治的扁桃体、腺样体肥大患儿80例,按随机数字表法分为两组,各40例。对照组按照常规沟通流程进行术前访视,观察组在对照组基础上按照CICARE沟通模式进行术前访视。比较两组焦虑状况、心率和收缩压水平、苏醒期谵妄情况及家属满意度。结果:观察组护理后改良耶鲁术前焦虑量表(m-YPAS)评分均低于对照组,差异均有统计学意义(P<0.05);观察组护理后心率、收缩压水平均低于对照组,差异均有统计学意义(P<0.05);观察组小儿麻醉苏醒期谵妄量表(PAED)评分均低于对照组,差异均有统计学意义(P<0.05);观察组家属满意度评分均高于对照组,差异均有统计学意义(P<0.05)。结论:在儿童扁桃体腺样体术前访视中采用CICARE沟通模式,可改善患儿焦虑状况,稳定心率、血压,减少苏醒期躁动发生,家属对护理服务满意。Objective:To explore the application effect of contact-introduction-communication-ask-respond-exit(CICARE)communication model in preoperative visits of tonsil adenoid in children.Method:A total of 80 children with tonsil and adenoid hypertrophy admitted to Jiangxi Provincial Children's Hospital from June 2021 to June 2023 were selected and divided into two groups according to random number table method,with 40 cases in each group.The control group conducted preoperative visit according to the routine communication procedure,and the observation group conducted preoperative visit according to the CICARE communication mode on the basis of the control group.The anxiety status,heart rate and systolic blood pressure levels,emergence delirium and family satisfaction were compared between two groups.Result:The scores of modified Yale preoperative anxiety scale(m-YPAS)in the observation group after nursing were lower than those in the control group,the differences were statistically significant(P<0.05).The heart rate and systolic blood pressure in the observation group after nursing were lower than those in the control group,the differences were statistically significant(P<0.05).The scores of pediatric anesthesia emergence delirium(PAED)in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The scores of family satisfaction in observation group were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:The application of CICARE communication mode in the preoperative visit of tonsil adenoid in children can improve the anxiety of children,stabilize the heart rate and blood pressure,reduce the occurrence of emergence delirium,and the families are satisfied with the nursing service.
