摘 要:目的:探讨脑卒中吞咽障碍患者实行不同舌骨上肌群加强训练方法的干预效果。方法:选取2022年12月—2023年12月蚌埠医科大学第一附属医院收治的90例脑卒中吞咽障碍患者进行研究,按照随机数字表法将患者分为各45例的两组。所有患者均给予常规吞咽训练,对照组给予基于舌骨上肌群的标准Shaker训练法,研究组给予基于舌骨上肌群的下颏抗阻力(CTAR)训练法。比较两组干预效果、吞咽功能恢复情况、电视透视检查吞咽试验(VFSS)运动学参数及生活质量。结果:研究组治疗总有效率较对照组高(P<0.05)。干预后,研究组标准吞咽功能评定量表(SSA)评分较对照组低,VFSS评分较对照组高(P<0.05)。干预后,研究组舌骨上移幅度、舌骨前移幅度及甲状软骨上移距离均高于对照组(P<0.05)。干预后,研究组生活质量各项评分均较对照组高(P<0.05)。结论:Shaker训练法和CTAR训练法均可使脑卒中患者吞咽障碍获得良好改善,但CTAR训练法的治疗总有效率更高,生活质量更优,实践价值较高。Objective:To investigate the intervention effect of different reinforcement training methods for suprahyoid muscle group in stroke patients with dysphagia.Method:A total of 90 stroke patients with dysphagia admitted to the First Affiliated Hospital of Bengbu Medical University from December 2022 to December 2023 were selected for this study.The patients were divided into two groups with 45 cases in each group according to random number table method.All patients were given routine swallowing training,the control group was given standard Shaker training based on suprahyoid muscle group,and the study group was given chin tuck against resistance(CTAR)training based on suprahyoid muscle group.The therapeutic effect,recovery of swallowing function,kinematic parameters of video-fluoroscopic swallowing study(VFSS)and quality of life were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group(P<0.05).After intervention,the standardized swallowing assessment(SSA)score of the study group was lower than that of the control group,and the VFSS score was higher than that of the control group(P<0.05).After intervention,the ascending range of hyoid bone,the ascending range of hyoid bone and the ascending distance of thyroid cartilage in the study group were higher than those in the control group(P<0.05).After intervention,the scores of quality of life in the study group were higher than those in the control group(P<0.05).Conclusion:Both Shaker training method and CTAR training method can improve the dysphagia of stroke patients,but CTAR training method has higher treatment efficiency,better quality of life and higher practical value.
