苓桂术甘汤联合艾灸疗法对慢性心力衰竭患者的疗效及miR-125b-5p、Th17/Treg的影响

摘　　要：目的:探究苓桂术甘汤联合艾灸疗法对慢性心力衰竭患者的疗效及miR-125b-5p、外周血辅助性T淋巴细胞17(Th17)/调节性T淋巴细胞(Treg)的影响。方法:选择2021年5月~2023年5月在本院治疗的186例慢性心力衰竭(CHF)患者作为研究对象,随机分为对照组( n =93)和联合组( n =93)。对照组给予常规西医干预+艾灸疗法,联合组给予常规西医干预+艾灸疗法+苓桂术甘汤,两组连续干预4w。统计分析两组临床疗效、治疗前、治疗4w后中医证候积分、心功能指标、6min步行距离变化、miR-125b-5p、Th17/Treg水平变化。结果:治疗后两组疗效等级有差异( P <0.05),且联合组有效率高于对照组[79.57% vs 66.66%]( P <0.05)。治疗前两组中医证候积分、心功能指标、6min步行距离变化、miR-125b-5p、Th17/Treg变化无差异( P >0.05);治疗后两组中医证候主症、次症积分、左室舒张末内径(LVED)、miR-125b-5p、Th17/Treg均下调,左室射血分数(LVEF)、6min步行距离均升高,且联合组更优( P <0.05)。结论:苓桂术甘汤联合艾灸疗法能有效治疗CHF,改善临床症状,且能降低miR-125b-5p、Th17/Treg水平,从而提升心功能。Objective: To explore the efficacy of Linggui Zhugan Decoction combined with moxibustion therapy on patients with chronic heart failure and the effects of miR-125b-5p and peripheral blood helper T lymphocyte 17(Th17) / regulatory T lymphocyte(Treg). Methods: A total of 186patients with chronic heart failure(CHF) who were treated in our hospital from May 2021to May 2023were randomly divided into control group( n =93) and combined group( n =93). The control group was given conventional western medicine intervention + moxibustion therapy, and the combined group was given conventional western medicine intervention + moxibustion therapy + Linggui Zhugan Decoction. The two groups were continuously intervened for 4weeks. The clinical efficacy, TCM syndrome scores, cardiac function indexes, 6-minute walking distance changes, miR-125b-5p and Th17/Treg levels before and after 4weeks of treatment were statistically analyzed. Results: After treatment, the efficacy levels of the two groups were different( P <0.05), and the effective rate of the combined group was higher than that of the control group [79.57% vs 66.66%]( P <0.05). There was no difference in TCM syndrome score, cardiac function index, 6-minute walking distance, miR-125b-5p and Th17/Treg between the two groups before treatment( P >0.05). After treatment, the scores of main symptoms and secondary symptoms of TCM syndromes, left ventricular end-diastolic diameter(LVED), miR-125b-5p and Th17/Treg were down-regulated, left ventricular ejection fraction(LVEF) and 6-minute walking distance were increased in both groups, and the combined group was better( P <0.05). Conclusion: Linggui Zhugan Decoction combined with moxibustion therapy can effectively treat CHF, improve clinical symptoms, and reduce the levels of miR-125b-5p and Th17/Treg, thereby improving cardiac function.
