化瘀温经汤结合桂枝茯苓胶囊治疗寒凝血瘀型子宫内膜异位症的疗效及对血清MMP-3、TGF-β、PGF2α、EMAb水平的影响

摘　　要：目的:观察化瘀温经汤结合桂枝茯苓胶囊治疗寒凝血瘀型子宫内膜异位症的疗效及对血清基质金属蛋白酶-3(MMP-3)、转化生长因子-β(TGF-β)、前列腺素F2α(PGF2α)以及抗子宫内膜抗体(EMAb)水平的影响。方法:回顾性分析2021年7月~2023年7月医院收治的78例寒凝血瘀型子宫内膜异位症患者的临床数据,根据治疗方案不同将患者分为对照组( n =37)及观察组( n =41)。对照组给予常规西药治疗,观察组在此基础上采用化瘀温经汤联合桂枝茯苓胶囊方案。对两组治疗前后的疼痛情况、生活质量、MMP-3、TGF-β、PGF2α及EMAb水平进行比较,并记录治疗期间患者不良反应发生率。结果:治疗后,对照组总有效率低于观察组( P <0.05);相较于对照组,治疗后观察组的简氏麦吉尔疼痛问卷[疼痛分级指数(PRI)、视觉模拟评分法(VAS)、现有疼痛强度(PPI)]及子宫内膜异位症生存质量量表(EHP-5)得分均显著降低( P <0.05);相较于对照组,治疗后观察组的血清MMP-3、TGF-β、PGF2α及EMAb水平均明显降低( P <0.05);对照组不良反应发生率与观察组比较,差异无统计学意义( P >0.05)。结论:化瘀温经汤结合桂枝茯苓胶囊方案对寒凝血瘀型子宫内膜异位症患者具有显著疗效,能够缓解疼痛,提升患者生活质量,调节血清MMP-3、TGF-β、PGF2α、EMAb水平,且具有较好的安全性。Objective: To observe the curative effect of Huayu Wenjing Decoction combined with Guizhi Fuling Glue in the treatment of endometriosis of cold coagulation and blood stasis type and its effect on the levels of serum matrix metalloproteinase-3(MMP-3), transforming growth factor-β(TGF-β), prostaglandin F2α(PGF2α) and anti-endometrial antibody(EMAb). Methods: The clinical data of 78patients with endometriosis of cold coagulation and blood stasis type admitted to our hospital from July 2021to July 2023were retrospectively analyzed. The patients were divided into reference group( n =37) and observation group( n =41) according to different treatment schemes. The reference group was treated with conventional western medicine, and the observation group was treated with Huayu Wenjing Decoction combined with Guizhi Fuling Capsule on this basis. The pain, quality of life, MMP-3, TGF-β, PGF2α and EMAb levels before and after treatment were compared between the two groups, and the incidence of adverse reactions during treatment was recorded. Results: After treatment, the total effective rate of the reference group was lower than that of the observation group( P <0.05). Compared with the reference group, the scores of Jane s McGill pain questionnaire [pain rating index(PRI), visual analogue scale(VAS), present pain intensity(PPI)] and endometriosis quality of life scale(EHP-5) in the observation group were significantly lower after treatment( P <0.05). Compared with the reference group, the levels of serum MMP-3, TGF-β, PGF2α and EMAb in the observation group were significantly lower after treatment( P <0.05). There was no significant difference in the incidence of adverse reactions between the reference group and the observation group( P >0.05). Conclusion: Huayu Wenjing Decoction combined with Guizhi Fuling Capsule has a significant effect on patients with endometriosis of cold coagulation and blood stasis type. It can relieve pain, improve the quality of life of patients, regulate the levels of serum MMP-3, TGF-β,
