摘　　要：目的探讨责任制护理模式用于重度急性呼吸窘迫综合征(ARDS)患者体外膜肺氧合(ECMO)治疗中的效果。方法选取医院2021年1月至2022年12月收治的重度ARDS患者78例,按随机数字表法分为观察组与对照组,各39例。对照组患者采取常规护理模式,观察组患者采取责任制护理模式。结果护理后,观察组呼吸衰竭消失时间、ECMO辅助时间、重症加强护理病房(ICU)入住时间显著短于对照组(P<0.05);氧合指数(PaO_(2)/FiO_(2))、呼吸系统顺应性(Crs)、用力肺活量(FVC)、第1秒用力呼气容积(FEV^(1))、FEV_(1)/FVC显著高于对照组(P<0.05);血管活性药物评分(VIS)及并发症发生率显著低于对照组(P<0.05)。结论重度ARDS患者予ECMO治疗期间采取责任制护理模式有助于预防多种并发症发生,缓解呼吸状态与肺功能,缩短恢复时间。Objective To investigate the application effect of accountable care mode in extracorporeal membrane oxygenation(ECMO)for patients with severe acute respiratory distress syndrome(ARDS).Methods A total of 78 patients with severe ARDS admitted to the hospital from January 2021 to December 2022 were selected and divided into the observation group and the control group according to the random number table method,with 39 cases in each group.The patients in the control group were given the conventional nursing mode,and the patients in the observation group were given the accountable care mode.Results After the nursing,the disappearance time of respiratory failure,ECMO-assisted time,and the time of admission to the intensive care unit(ICU)in the observation group was significantly shorter than that in the control group(P<0.05);the oxygenation index(PaO_(2)/FiO_(2)),respiratory system compliance(Crs),forceful lung volume(FVC),forceful expiratory volume in the first second(FEV_(1)),and FEV_(1)/FVC were significantly higher than those in the control group(P<0.05);the vasoactive-inotropic score(VIS)and incidence of complication were significantly lower than those in the control group(P<0.05).Conclusion The application of accountable care mode during ECMO treatment for patients with severe ARDS can help prevent multiple complications,relieve respiratory status and lung function,and shorten recovery time.
