内镜下痔套扎术联合透明帽辅助下消痔灵注射术治疗Ⅰ~Ⅲ度内痔的效果

摘　　要：目的:分析内镜下痔套扎术联合透明帽辅助下消痔灵注射术治疗Ⅰ~Ⅲ度内痔的效果。方法:选取赣州市中医院肛肠科2022年1月—2023年1月收治的90例Ⅰ~Ⅲ度内痔患者作为研究对象,按照随机数字表法分为试验组(30例)、对照组1(30例)、对照组2(30例),试验组采用内镜下痔套扎术联合透明帽辅助下消痔灵注射术,对照组1采用单纯内镜下痔套扎,对照组2采用肛门镜下内痔结扎术联合消痔灵内痔硬化剂注射术。比较三组症状评分、并发症的发生率、住院时间、复发率。结果:试验组治疗后3、7、14 d的便血、痔核脱出、瘙痒、坠胀、疼痛评分均低于对照组1,差异均有统计学意义(P<0.05)。三组的并发症发生率比较,差异无统计学意义(P>0.05)。试验组住院时间短于对照组2,差异有统计学意义(P<0.05)。试验组复发率低于对照组1,差异有统计学意义(P<0.05)。结论:内镜下痔套扎术联合透明帽辅助下消痔灵注射术治疗Ⅰ~Ⅲ度内痔,可改善临床症状,缩短住院时间,降低复发率,且安全性良好。Objective:To analyze the effect of endoscopic hemorrhoidal ligation combined with transparent cap assisted injection of Xiaozhiling in the treatment of gradeⅠ-Ⅲinternal hemorrhoids.Method:A total of 90 patients with gradeⅠ-Ⅲinternal hemorrhoids admitted to Ganzhou Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were selected as research objects,and were divided into experimental group(30 cases),control group 1(30 cases)and control group 2(30 cases)according to random number table.The experimental group received endoscopic hemorrhoidal ligation combined with transparent cap assisted injection of Xiaozhiling,the control group 1 was treated with simple endoscopic hemorrhoidal ligation,the control group 2 was treated with anoscopic internal hemorrhoidal ligation combined with Xiaozhiling internal hemorrhoidal hardener injection.Symptom score,incidence of complications,length of stay and recurrence rate were compared among the three groups.Result:The scores of hematochezia,hemorrhoidal prolapse,pruritus,distension and pain in the experimental group were lower than those in the control group 1 at 3,7 and 14 days after treatment,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications among the three groups(P>0.05).The hospitalization time of experimental group was shorter than that of control group 2,the difference was statistically significant(P<0.05).The recurrence rate of experimental group was lower than that of control group 1,the difference was statistically significant(P<0.05).Conclusion:Endoscopic hemorrhoidal ligation combined with transparent cap assisted injection of Xiaozhiling for the treatment of gradeⅠ-Ⅲinternal hemorrhoids can improve clinical symptoms,shorten hospital stay,reduce recurrence rate,and have good safety.
