针刺疗法联合补中益气汤加减对子宫颈癌根治术后尿潴留患者膀胱功能恢复的影响

摘　　要：目的:探究针刺疗法联合补中益气汤加减对子宫颈癌根治术后尿潴留患者膀胱功能恢复的影响。方法:按随机数字表法将2021年9月—2023年9月在江西省妇幼保健院就诊的子宫颈癌根治术后尿潴留患者120例分为对照组与观察组,各60例。所有患者均常规留置导尿,并予以物理刺激疗法。对照组进行针刺疗法,观察组在对照组基础上联合补中益气汤加减。比较两组临床疗效、膀胱功能恢复情况,以及中医症候积分、尿动力学参数[最大尿流率(Qmax)、最大逼尿肌压力、最大膀胱容量]和不良事件发生情况。结果:观察组总有效率高于对照组,自主排尿恢复时间早于对照组,平均排尿时间短于对照组,差异均有统计学意义(P<0.05)。治疗2周后,两组中医症候积分较治疗前均降低,且观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗2周后,观察组尿动力学参数均大于对照组,差异均有统计学意义(P<0.05)。结论:针刺疗法联合补中益气汤加减治疗子宫颈癌根治术后尿潴留患者,能够有效改善患者症状,提高尿动力学参数,快速改善膀胱功能。Objective:To explore the effect of acupuncture therapy combined with Modified Buzhong Yiqi Decoction on the recovery of bladder function in patients with urinary retention after radical resection of cervical cancer.Method:A total of 120 patients with urinary retention after radical resection of cervical cancer treated in Jiangxi Maternal and Child Health Hospital from September 2021 to September 2023 were divided into a control group and an observation group according to the random number table method,with 60 cases in each group.All patients underwent routine indwelling catheterization and received physical stimulation therapy.The control group received acupuncture therapy,and the observation group was combined with Modified Buzhong Yiqi Decoction on the basis of the control group.The clinical efficacy,recovery of bladder function,TCM symptoms scores,urodynamic parameters[maximum flow rate(Qmax),maximal detrusor pressure,maximal bladder capacity]and adverse events were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the recovery time of spontaneous urination was earlier than that of the control group,average urination time was shorter than that of the control group,the differences were statistically significant(P<0.05).After 2 weeks of treatment,TCM syndromes scores in both groups were lower than those before treatment,and those in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After 2 weeks of treatment,the urodynamic parameters in observation group were higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:Acupuncture therapy combined with Modified Buzhong Yiqi Decoction can effectively improve the symptoms of patients with urinary retention after radical resection of cervical cancer,improve the parameters of urodynamics,and quickly improve the bladder function.
