经皮撬拨闭合复位克氏针内固定与切开复位内固定治疗跟骨骨折的效果比较

摘　　要：目的:对比跟骨骨折患者采用切开复位内固定与经皮撬拨闭合复位克氏针内固定治疗的效果。方法:选择2020年1月—2023年1月中国融通医疗健康集团鹰潭一八四医院收治的60例跟骨骨折患者作为研究对象,按照随机数字表法进行分组,各30例。对照组采用切开复位内固定治疗,试验组采用经皮撬拨闭合复位克氏针内固定治疗。比较两组疗效、Gissane角、Bohler角、跟骨宽度、跟骨高度及并发症发生情况。结果:两组优良率比较,差异无统计学意义(P>0.05);两组术后1年Gissane角、Bohler角均较术前增大,跟骨高度均较术前增高,跟骨宽度均较术前下降(P<0.05);两组术后1年Gissane角、Bohler角、跟骨高度、跟骨宽度比较,差异均无统计学意义(P>0.05);试验组并发症发生率(6.66%)低于对照组(30.00%)(P<0.05)。结论:经皮撬拨闭合复位克氏针内固定与切开复位内固定治疗跟骨骨折均有良好的效果,两种术式均可在一定程度上改善Gissane角、Bohler角、跟骨高度、跟骨宽度,但前者可显著减少患者术后并发症发生情况。Objective:To compare the effect of percutaneous prying closed reduction Kirschner wire internal fixation and open reduction and internal fixation in patients with calcaneous fracture.Method:A total of 60 patients with calcaneous fracture admitted to Yingtan 184th Hospital,China Rongtong Medical Health Group from January 2020 to January 2023 were selected as the study objects,and they were divided into two groups according to the random number table method,with 30 cases in each group.The control group was treated with open reduction and internal fixation,and the experimental group was treated with percutaneous prying closed reduction Kirschner wire internal fixation.The curative effect,Gissane angle,Bohler angle,calcaneus width,calcaneus height and complications were compared between the two groups.Result:There was no significant difference in the excellent and good rate between the two groups(P>0.05).1 year after surgery,compared with before surgery,Gissane angle and Bohler angle increased,calcaneus height increased and calcaneus width decreased in both groups(P<0.05).There were no significant differences in Gissane angle,Bohler angle,calcaneus width and calcaneus height between the two groups 1 year after surgery(P>0.05).The complication rate of experimental group(6.66%)was lower than that of control group(30.00%)(P<0.05).Conclusion:Both percutaneous prying closed reduction Kirschner wire internal fixation and open reduction and internal fixation have good results in the treatment of calcaneous fracture.Both methods can improve Gissane angle,Bohler angle,calcaneus height and calcaneus width to a certain extent,but the former can significantly reduce the incidence of postoperative complications.
