沙库巴曲缬沙坦钠对原发性高血压患者RAAS的影响及肾脑保护作用

摘　　要：目的:探究沙库巴曲缬沙坦钠对原发性高血压患者肾素-血管紧张素-醛固酮系统(RAAS)的影响及肾脑保护作用。方法:将2022年1月—2023年9月福州市第二总医院收治的180例原发性高血压患者依据随机数字表法分为对照组(n=90)和观察组(n=90)。对照组采用缬沙坦进行治疗,观察组采用沙库巴曲缬沙坦钠进行治疗。比较两组血压达标率、不良反应发生率、RAAS指标[血浆肾素活性(PRA)、醛固酮(ALD)及血管紧张素Ⅱ(AngⅡ)]、肾血流指标[叶间动脉平均血流速(Vm)、搏动指数(PI)、阻力指数(RI)]、肾功能指标[血肌酐(Scr)、血尿素氮(BUN)]及脑血流指标(大脑中动脉Vm、PI及RI)。结果:观察组血压达标率显著高于对照组,差异有统计学意义(P<0.05);两组各种不良反应发生率比较,差异均无统计学意义(P>0.05)。治疗前两组RAAS指标、肾血流指标、肾功能指标及脑血流指标比较,差异均无统计学意义(P>0.05)。治疗4、8周后,观察组RAAS指标、肾功能指标、叶间动脉及大脑中动脉PI及RI均显著低于对照组,叶间动脉及大脑中动脉Vm均显著高于对照组,差异均有统计学意义(P<0.05)。结论:沙库巴曲缬沙坦钠在原发性高血压患者中的疗效较好,可显著改善RAAS,具有较好的肾脑保护作用。Objective:To investigate the effect of Sacubitril Valsartan Sodium on renin angiotensin aldosterone system(RAAS)in patients with essential hypertension and its protective effects on kidney and brain.Method:A total of 180 patients with essential hypertension admitted to Fuzhou Second General Hospital from January 2022 to September 2023 were divided into control group(n=90)and observation group(n=90)according to random number table method.The control group was treated with Valsartan,and the observation group was treated with Sacubitril Valsartan Sodium.The blood pressure control rate,incidence of adverse reactions,RAAS indexes[plasma renin activity(PRA),aldosterone(ALD)and angiotensinⅡ(AngⅡ)],renal blood flow indexes[mean velocity(Vm),pulse index(PI),resistance index(RI)of interlobar artery],renal function indexes[serum creatinine(Scr),blood urea nitrogen(BUN)]and cerebral blood flow indexes(Vm,PI and RI of middle cerebral artery)of two groups were compared.Result:The blood pressure control rate of observation group was significantly higher than that of control group,the difference was statistically significant(P<0.05).There were no significant differences in the incidence of various adverse reactions between the two groups(P>0.05).There were no significant differences in RAAS indexes,renal blood flow indexes,renal function indexes and cerebral blood flow indexes between two groups before treatment(P>0.05).After 4 and 8 weeks of treatment,RAAS indexes,renal function indexes,PI and RI of interlobar artery and middle cerebral artery in observation group were significantly lower than those in control group,and Vm of interlobar artery and middle cerebral artery were significantly higher than those in control group,the differences were statistically significant(P<0.05).Conclusion:Sacubitril Valsartan Sodium has a good curative effect in patients with essential hypertension,which can significantly improve RAAS and has a good protective effect on kidney and brain.
