摘 要:目的:尿α1微球蛋白(α1-MG)、尿β2微球蛋白(β2-MG)、尿微量白蛋白/肌酐比(ACR)联合检测在早期糖尿病肾病(EDN)诊断中的意义。方法:选取2022年11月—2023年12月在盐城市中医院住院的208例2型糖尿病患者作为研究对象,根据患者24 h尿蛋白定量水平(UAER),将单纯糖尿病(DM)组、EDN组和临床糖尿病肾病(DN)组患者作为研究组,另选取43例同期在盐城市中医院健康体检者为对照组。比较两组的尿α1-MG、β2-MG和ACR检测结果差异,比较研究组的尿α1-MG、β2-MG和ACR检测结果差异,绘制受试者工作曲线(ROC)分析3项指标单独及联合检测对糖尿病患者早期肾损伤(EKI)的诊断意义。结果:研究组的尿α1-MG、β2-MG和ACR水平均高于对照组,EDN组的尿α1-MG、β2-MG和ACR水平均明显高于较DM组,DN组的3项指标均较EDN组明显增高,差异有统计学意义(P<0.05);ROC曲线显示,尿α1-MG、β2-MG和ACR单独与联合检测预测2型糖尿病发生早期肾损伤的AUC分别为0.833、0.870、0.981、0.993,其中联合检测预测价值更高。结论:尿α1-MG、β2-MG和ACR联合检测在诊断EDN中具有较高的临床意义。Objective:To investigate the significance of urinaryα1 microglobulin(α1-MG),urinaryβ2 microglobulin(β2-MG)and urinary microalbumin/creatinine ratio(ACR)combined detection in early-stage diabetic nephropathy(EDN).Method:A total of 208 patients with type 2 diabetes who were hospitalized in Yancheng Hospital of Traditional Chinese Medicine from November 2022 to December 2023 were selected as the study objects.According to the 24-hour urinary albumin ejection rate(UAER),the patients were divided into diabetes mellitus(DM)group,EDN group and clinical diabetic nephropathy(DN)group.These three groups are study groups.Another 43 healthy subjects in Yancheng Hospital of Traditional Chinese Medicine were selected as control group.The differences in urineα1-MG,β2-MG and ACR detection results between the two groups were compared,and the differences in urineα1-MG,β2-MG and ACR detection results between the study group were compared,and receiver operating curve(ROC)was drawn to analyze the diagnostic significance of 3 indicators alone and combined detection in early kidney injury in diabetic patients.Result:Urinaryα1-MG,β2-MG and ACR levels in study group were higher than those in control group,urinaryα1-MG,β2-MG and ACR levels in EDN group were significantly higher than those in DM group,and the three indexes in DN group were significantly higher than those in EDN group,and the differences were statistically significant(P<0.05).ROC curve showed that the AUC ofα1-MG,β2-MG and ACR alone and combined detection in predicting early kidney injury in type 2 diabetes mellitus were 0.833,0.870,0.981 and 0.993,respectively,and the combined detection had a higher predictive value(P<0.05).Conclusion:Urineα1-MG,β2-MG and ACR combined detection has high clinical value in early diagnosis of diabetic nephropathy.
