摘 要:目的:研究不同剂量布地奈德吸入治疗对肺发育不全早产儿动脉血气指标及预后的影响。方法:选取2020年1月—2024年1月石狮市妇幼保健院出生的60例早产合并肺发育不全患儿作为研究对象,按照出生顺序编号后,以单双号随机分为A组、B组,每组各30例。两组在常规治疗基础上分别进行0.5 mg/d、1.0 mg/d布地奈德药物雾化吸入治疗。比较两组治疗前及治疗2周后动脉血气指标及肺功能变化,8周后评估两组生长发育及神经发育情况。结果:治疗2周后,B组血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))改善情况优于A组,差异有统计学意义(P<0.05);治疗2周后,A组呼吸频率(RR)高于B组,潮气量(TV)、达峰容积(VPEF)低于B组,差异有统计学意义(P<0.05);8周后两组身长、体重及神经发育情况比较,差异无统计学意义(P>0.05)。结论:相比0.5 mg/d的布地奈德吸入治疗,1.0 mg/d的治疗对肺发育不全早产儿动脉血气指标及肺功能改善效果好,且不影响其预后体格及神经发育。Objective:To study the effect of inhalation of different doses of budesonide on arterial blood gas parameters and prognosis in preterm infants with pulmonary hypoplasia.Method:A total of 60 children born in Shishi Maternal and Child Health Hospital from January 2020 to January 2024 were selected as the study objects.After being numbered according to birth order,they were randomly divided into group A and Group B by oddy-even numbers,with 30 cases in each group.The two groups were treated with budesonide aerosol inhalation at 0.5 mg/d and 1.0 mg/d on the basis of conventional treatment,respectively.The changes of arterial blood gas indexes and lung function were compared between the two groups before and after 2 weeks of treatment.The growth and neurodevelopment of the two groups were evaluated after 8 weeks.Result:After 2 weeks of treatment,the improvement of blood oxygen partial pressure(PaO_(2))and arterial blood carbon dioxide partial pressure(PaCO_(2))in group B was better than that in group A,and the difference was statistically significant(P<0.05).After 2 weeks of treatment,respiratory rate(RR)in group A was higher than that in group B,tidal volume(TV)and peak volume(VPEF)were lower than that in group B,and the differences were statistically significant(P<0.05).After 8 weeks,there was no significant difference in body length,body weight and neurodevelopment between the two groups(P>0.05).Conclusion:Compared with 0.5 mg/d of budesonide inhalation,1.0 mg/d of budesonide inhalation therapy has a better effect on arterial blood gas indexes and lung function in premature infants with pulmonary hypoplasia,and does not affect the prognosis of physical and neurological development.
