司库奇尤单抗联合窄谱紫外线治疗进展期斑块型银屑病的临床效果分析

摘　　要：目的:探讨司库奇尤单抗联合窄谱紫外线治疗进展期斑块型银屑病的效果。方法:选择2022年1月—2023年6月海安市皮肤病医院收治的70例进展期斑块型银屑病患者;按照随机数字表法分成两组,各35例。两组均给予阿维A胶囊和他克莫司软膏治疗,在此基础上,对照组给予窄谱紫外线光疗,观察组给予司库奇尤单抗联合窄谱紫外线治疗。比较两组治疗前后皮损的血管球直径、银屑病面积与严重性指数(PASI)评分(患者均行皮肤镜检查),血清白细胞介素-17(IL-17)、肿瘤坏死因子-α(TNF-α)和CD4^(+)/CD8^(+)比值,以及治疗效果和不良反应发生情况。结果:治疗6、12周后,两组血管球直径和PASI评分均低于治疗前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗12周后,两组IL-17、TNF-α均低于治疗前,CD4^(+)/CD8^(+)均高于治疗前,且观察组IL-17、TNF-α均低于对照组,CD4^(+)/CD8^(+)高于对照组,差异均有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。两组不良反应率比较,差异无统计学意义(P>0.05)。结论:司库奇尤单抗联合窄谱紫外线光疗能改善进展期斑块型银屑病患者的炎症免疫状态,提高治疗效果,皮肤镜能了解治疗进展情况。Objective:To investigate the effect and value of Secukinumab combined with narrow spectrum ultraviolet in the treatment of advanced plaque psoriasis.Method:A total of 70 patients with advanced plaque psoriasis admitted to Hai'an Dermatology Hospital from January 2022 to June 2023 were selected.According to the random number table method,the patients were divided into two groups,with 35 cases in each group.Both groups were treated with Acitretin Capsules and Tacrolimus Ointment.On this basis,the control group was treated with narrow spectrum ultraviolet phototherapy,and the observation group was treated with Secuchiumab combined with narrow spectrum ultraviolet therapy.The vascular sphere diameter,psoriatic area and severity index(PASI)score(dermoscope was performed in all patients),serum interleukin-17(IL-17),tumor necrosis factor-α(TNF-α)and CD4^(+)/CD8^(+)ratio before and after treatment,as well as the treatment effect and the occurrence of adverse reactions between the two groups were compared.Result:After 6 and 12 weeks of treatment,vascular bulb diameter and PASI scores in two groups were lower than those before treatment,and those in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,levels of IL-17 and TNF-αin both groups were lower than those before treatment,and levels of CD4^(+)/CD8^(+)were higher than those before treatment,and levels of IL-17 and TNF-αin observation group were lower than those in control group,and level of CD4^(+)/CD8^(+)was higher than that in control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the rate of adverse reactions between the two groups(P>0.05).Conclusion:Secuchiumab combined with narrow spectrum ultraviolet phototherapy can improve the inflammatory and immune status of patients with a
