黄芪桂枝五物汤加味联合穴位注射治疗气虚血瘀型糖尿病性周围神经病变的临床观察

摘　　要：目的:探讨气虚血瘀型糖尿病性周围神经病变(DPN)患者应用黄芪桂枝五物汤加味联合穴位注射治疗的效果。方法:采用随机数字表法将94例景德镇市中医医院2022年1月—2023年6月收治的气虚血瘀型DPN患者分为两组。采用穴位注射治疗的47例患者设为对照组,采用穴位注射+黄芪桂枝五物汤加味治疗的47例患者设为观察组,均连续治疗4周。比较两组临床疗效、临床症状严重程度、神经传导速度、肿瘤坏死因子-α(TNF-α)水平和不良反应。结果:观察组治疗总有效率高于对照组,治疗后多伦多临床评分系统(TCSS)评分及TNF-α水平均较对照组低,胫神经、腓总神经、正中神经感觉神经传导速度(SNCV)和运动神经传导速度(MNCV)均较对照组高(P<0.05);两组患者均未见明显不良反应。结论:在穴位注射治疗基础上,联合黄芪桂枝五物汤加味治疗气虚血瘀型DPN可明显提高治疗效果,改善神经传导速度,降低TNF-α水平,促进患者临床症状改善,是一种安全、有效的治疗方案。Objective:To investigate the effect of Modified Huangqi Guizhi Wuwu Decoction combined with acupoint injection in the treatment of diabetes peripheral neuropathy(DPN)of Qi deficiency and blood stasis type.Method:A total of 94 patients with DPN of Qi deficiency and blood stasis type admitted to Jingdezhen Traditional Chinese Medicine Hospital from January 2022 to June 2023 were divided into two groups using a random number table method.47 patients treated with acupoint injection were set as the control group,47 patients treated with acupoint injection and modified Huangqi Guizhi Wuwu Decoction were set as the observation group,patients in two groups were treated continuously for 4 weeks.The clinical efficacy,severity of clinical symptoms,nerve conduction velocity,and tumor necrosis factor-α(TNF-α)level and adverse reactions between two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group,the Toronto clinical scoring system(TCSS)score and TNF-αafter treatment in the observation group were lower than those of the control group,while the sensory nerve conduction velocity(SNCV)and motor nerve conduction velocity(MNCV)of tibial nerve,common peroneal nerve and median nerve were higher than those of the control group(P<0.05).There were no significant adverse reactions in both groups of patients.Conclusion:On the basis of acupoint injection therapy,the combination of Modified Huangqi Guizhi Wuwu Decoction treatment for DPN of Qi deficiency and blood stasis type can significantly improve the therapeutic effect,improve nerve conduction velocity,and reduce TNF-αlevel,promoting the improvement of clinical symptoms in patients,is a safe and effective treatment plan.
