摘　　要：目的:探讨烧伤皮肤愈合后瘢痕患儿采用脉冲染料激光(PDL)联合硅酮凝胶治疗的临床效果。方法:采用随机对照研究方案,研究时间为2019年1月—2023年12月,共选取江南大学附属儿童医院收治的80例烧伤皮肤愈合后瘢痕患儿,分组方法为随机数字表法。对照组(n=40)患儿治疗方案为PLD,观察组(n=40)患儿治疗方案为PDL联合硅酮凝胶。比较两组临床总有效率、温哥华瘢痕量表(VSS)、增生性瘢痕面积、视觉模拟评分法(VAS)、数字分级评分法(NRS)、不良反应发生率。结果:观察组临床总有效率高于对照组,差异有统计学意义(P<0.05);治疗后观察组VSS评分均低于对照组,差异均有统计学意义(P<0.05);治疗后观察组增生性瘢痕面积小于对照组,VAS评分、NRS评分均低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:烧伤皮肤愈合后瘢痕患儿采用PDL联合硅酮凝胶治疗的效果显著,可改善瘢痕区域皮肤状态,缩小增生性瘢痕面积,缓解瘢痕区域瘙痒及疼痛症状,治疗安全性较高。Objective:To explore the clinical effect of pulsed dye laser(PDL)combined with silicone gel in the treatment of scar after burn skin healing in children.Method:A randomized controlled study scheme was adopted,and the study period was from January 2019 to December 2023.A total of 80 children with scar after burn skin healing admitted to Affiliated Children's Hospital of Jiangnan University were selected,and the grouping method was random number table method.The control group(n=40)was treated with PLD,and the observation group(n=40)was treated with PDL combined with silicone gel.The clinical total effective rate,Vancouver scar scale(VSS),hyperplastic scar area,visual analogue scale(VAS),numerical rating scale(NRS)and incidence of adverse reactions were compared between the two groups.Result:The total clinical effective rate of observation group was higher than that of control group,the difference was statistically significant(P<0.05).After treatment,the VSS scores of observation group were lower than those of control group,the differences were statistically significant(P<0.05).After treatment,the hyperplastic scar area in observation group was smaller than that in control group,and the scores of VAS and NRS were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The effect of PDL combined with silicone gel in the treatment of children with scar after burn skin healing is remarkable,which can improve the skin condition of scar area,reduce the area of hyperplastic scar,relieve itching and pain symptoms in scar area,and the treatment safety is high.
