五灵胶囊预防肺结核患者药物性肝损伤的效果及对免疫功能的影响

摘　　要：目的:观察五灵胶囊预防肺结核患者药物性肝损伤的效果及对免疫功能的影响。方法:选取赣州市第五人民医院2020年1月—2023年8月收治的80例肺结核患者,采用随机数字表法分为对照组(n=40,常规抗结核药物及葡醛内酯治疗)和观察组(n=40,常规抗结核药物联合五灵胶囊治疗)。比较两组肝损伤发生情况、肝功能指标、免疫功能及不良反应发生情况。结果:治疗后,观察组丙氨酸氨基转移酶(ALT)、γ-谷氨酰转移酶(GGT)、天门冬氨酸氨基转移酶(AST)及对照组总胆红素(TBIL)、碱性磷酸酶(ALP)、ALT、GGT、AST均较治疗前升高,但观察组TBIL、ALT、GGT、ALP及AST水平均低于对照组(P<0.05);观察组肝损伤总发生率显著低于对照组(P<0.05);治疗后,观察组CD4^(+)、CD3^(+)均较治疗前升高,观察组均高于对照组,CD8^(+)较治疗前降低,观察组低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:五灵胶囊对肺结核患者药物性肝损伤具有预防效果,且能有效提升患者免疫功能。Objective:To observe the effect of Wuling Capsules on preventing drug-induced liver injury in patients with pulmonary tuberculosis and its influence on im mune function.Method:A total of 80 patients with pulmonary tuberculosis admitted to the Fifth People's Hospital of Ganzhou from January 2020 to August 2023 were divided into control group(n=40,treated with conventional anti-tuberculosis drugs and Glucurolactone)and observation group(n=40,treated with conventional anti-tuberculosis drugs combined with Wuling Capsules)according to random number table method.The incidence of liver injury,liver function indexes,immune function and adverse reactions were compared between the two groups.Result:After treatment,alanine aminotransferase(ALT),γ-glutamyl transferase(GGT)and aspartate aminotransferase(AST)in the observation group and total bilirubin(TBIL),alkaline phosphatase(ALP),ALT,GGT and AST in the control group were increased,but the levels of TBIL,ALT,GGT,ALP and AST in observation group were lower than those in control group(P<0.05).The total incidence of liver injury in observation group was significantly lower than that in control group(P<0.05).After treatment,CD4^(+)and CD3^(+)in observation group were higher than those before treatment,and those in observation group were higher than those in control group,CD8^(+)was lower than that before treatment,and that in observation group was lower than that in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Wuling Capsules has preventive effect on drug-induced liver injury in patients with pulmonary tuberculosis,and can effectively improve the immune function of patients.
