益气活血汤对气滞血瘀型肩袖损伤患者行关节镜下肩袖修补术后肩关节功能恢复的影响

摘　　要：目的:探讨益气活血汤对气滞血瘀型肩袖损伤患者关节镜下肩袖修补术后肩关节功能恢复的影响。方法:选取2019年6月—2023年6月宝鸡市中医医院收治的90例气滞血瘀型肩袖损伤患者,随机分为观察组和对照组,每组45例。两组患者均进行关节镜下肩袖修补术治疗,对照组术后给予常规康复治疗干预,观察组增加益气活血汤治疗。评估两组间临床疗效、中医证候积分、肩关节活动度、炎症因子(基质金属蛋白酶13、软骨寡聚基质蛋白、单核细胞趋化蛋白4)、肩关节功能恢复效果(洛杉矶加州大学关节功能评分)、治疗安全性。结果:观察组的总有效率高于对照组(P<0.05)。治疗后,观察组中医证候积分及炎症因子水平低于对照组(P<0.05),观察组肩关节活动度及肩关节功能恢复效果均优于对照组(P<0.05),两组不良反应发生率差异无统计学意义(P>0.05)。结论:益气活血汤对具有气滞血瘀的肩袖损伤患者治疗效果良好,可以提高肩关节活动度,抑制炎症因子表达,恢复患者的肩关节活动功能,治疗安全性良好。Objective To investigate the effect of Yiqi Huoxue decoction on the recovery of shoulder joint function after arthroscopic rotator cuff repair surgery in patients with qi stagnation and blood stasis type rotator cuff injury.Methods 90 patients with rotator cuff injury of Qi stagnation and blood stasis type were randomly divided into observation group and control group by random number table method,with 45 cases in each group.Both groups of patients underwent arthroscopic rotator cuff repair surgery.The control group received routine rehabilitation intervention after surgery,while the observation group received additional treatment with Yiqi Huoxue decoction.The clinical efficacy,traditional Chinese medicine syndrome score,shoulder joint range of motion,inflammatory factor(MMP-13,COMP,MCP-4),shoulder joint function recovery effect(UCLA joint function score),and treatment safety were evaluated between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the traditional Chinese medicine syndrome score,inflammatory factor were lower than the control group(P<0.05).The range of motion of the shoulder joint and shoulder joint function recovery effect in the observation group were higher than that in the control group in terms of abductio(P<0.05).The incidence of adverse reactions between two groups with no statistically significant difference(P>0.05).Conclusion Yiqi Huoxue decoction has a good therapeutic effect on patients with Qi stagnation and blood stasis type after arthroscopic rotator cuff repair surgery.It can improve shoulder joint mobility,inhibit the expression of inflammatory factors,restore shoulder joint mobility,and ensure good treatment safety.
