摘　　要：目的系统检索国内外完全植入式输液港(TIVAP)患者导管相关感染的危险因素并进行数据整合。方法检索中国知网、万方数据知识服务平台、维普中国科技期刊数据库、中国生物医学文献数据库、PubMed、Web of science、Embase数据库。对纳入文献进行质量评价并提取数据,使用Review Manager 5.3对数据进行Meta分析。结果共检索到173篇文献,最终纳入17篇进行Meta分析,纳入文献总计样本量9518例,肿瘤完全植入式输液港并发导管相关感染的发生率在2.2%~50%。合并糖尿病(OR=5.02,95%CI:3.42~7.36)、进行肠外营养支持(OR=2.22,95%CI:1.59~3.10)及接受姑息治疗(OR=3.57,95%CI:1.86~6.84)是肿瘤患者TIVAP并发感染的危险因素。结论影响肿瘤患者TIVAP相关感染的危险因素较多,该研究结果有利于临床护理人员识别植入式输液港导管感染并发症相关风险因素,精准临床管理,进而改善患者的生活质量及预后。Objective To identify the risk factors of catheter-related infections in patients with Totally Implanted Venous Access Port(TIVAP)by a Meta-analysis.Methods Computerized search was performed for studies on the risk factors of catheter-related infections in patients with TIVAP in the CNKI,Wanfang Data,VIP CBM,PubMed,Web of science and Embase database.The quality assessment and data extraction were performed,and Meta-analysis was performed by Review Manager 5.3.Results A total of 173 literatures were retrieved,and 17 were included for Meta-analysis.The total sample size of the literatures included 9518 cases.The incidence rate of catheter-related infection was from 2.2%to 50%at the infusion port of complete tumor implantation.Diabetes mellitus(OR=5.02,95%CI:3.42-7.36),parenteral nutrition support(OR=2.22,95%CI:1.59-3.10)and palliative care(OR=3.57,95%CI:1.86-6.84)were risk factors for TIVAP infection in cancer patients.Conclusions There are many risk factors involved in TIVAP-related infection in cancer patients,and clinical nurses should identify risk factors related to complications of implantable infusion port catheter infection and precise clinical management,so as to improve the quality of life and outcome.
