FOCUS-PDCA模式用于重力输液中长回血管理的效果分析

摘　　要：目的探讨FOCUS-PDCA模式解决患者在重力输液过程中常见回血因素的效果。方法针对临床上常见的重力输液过程中,因如厕、下床等因素引起的输液长回血现象进行干预,比较在FOCUS-PDCA模式管理前(2022年6月1日至10月30日)763例3821次输液,管理后(2022年11月1日至2023年3月30日)811例患者3923次输液,其中出现的输液长回血情况以及输液长回血后导致的输液堵管、肿胀、静脉炎、输液不畅、外渗等情况的发生率以及回血相关的患者投诉情况。结果FOCUS-PDCA模式干预后,患者回血率从11.66%下降至2.34%;干预前后,患者长回血相关并发症即回血相关并发症差异有统计学意义(P<0.05)。结论FOCUS-PDCA模式降低了患者在重力输液过程中的长回血现象以及因此带来的堵管、肿胀、静脉炎、输液不畅、外渗等情况,减少了医疗投诉,提高了患者就医体验。Objective To explore the effect of the FOCUS-PDCA model in addressing common factors contributing to blood reflux during gravity intravenous infusion.Methods To intervene for the long blood feflux during gravity intravenous infusion caused by patient movement,such as toileting,getting out of bed.The study compared the incidence of prolonged blood reflux and its related complications,such as infusion blockage,swelling,phlebitis,infusion incompetence,extravasation-before(June 1,2022-October 30,2022)and after FOCUS-PDCA model management(Nov.1,2022-March 30,2023)implementing the FOCUS-PDCA model.The comparison included 763 cases with 3821 infusions instacnes before the intervention and 811 patients with 3923 infusions instances after the intervention,and the incidence of patient complaints related to blood reflux was analyzed.Results After the implementation of the FOCUS-PDCA model,the rate of patient blood reflux decreased significantly from 11.66%to 2.34%(P<0.05).Conclusions The FOCUS-PDCA model could reduce the occurrence of prolonged blood reflux during gravity intravenous infusion as well as the associated complications such as blockage of infusion tubes,swelling,phlebitis,poor infusion flow,and extravasation,but also this model could reduce medical complaints,and improve the patient experience and satifaction with medical services.
