摘　　要：目的分析老年帕金森病(Parkinson’s disease,PD)患者中出现重度以下年龄相关性听力损失(agerelated hearing loss,ARHL)的听力学特征和血浆长链非编码RNA(Long non-coding RNA,lncRNA)的表达差异。方法纳入2022年1月至2023年5月就诊于中国人民解放军总院的重度以下ARHL患者15例。根据是否罹患PD分为PD组13例,非PD组2例。15例患者均通过听力学测试,行全转录组RNA测序,根据听力损失程度、性别、年龄,分析其听力学和血浆中lnc RNA的差异表达特征。结果男性各频率平均听阈与女性比较,差异无统计学意义(P>0.05)。60~69岁各频率平均听阈与70~79岁比较,差异无统计学意义(P>0.05)。Ⅴ波引出率最高,Ⅰ波最低,相同性别,Ⅰ、Ⅲ、Ⅴ波引出率比较,差异均无统计学意义(P>0.05)。不同听力损失程度各波潜伏期及波间期比较,差异无统计学意义(P>0.05)。PD组70~79岁与60~69岁RNA-seq差异表达的lncRNA472个,上调239个,下调233个;PD组女性与男性RNA-seq差异表达的lncRNA535个,上调458个,下调77个;PD组轻度听力损失、中度听力损失RNA-seq差异表达的lncRNA550个,上调107个,下调443个;PD组中度听力损失与中重度听力损失差异表达的lncRNA1115个,上调226个,下调889个。结论PD患者的听力损失程度、性别和年龄,听力学检测无显著差异,但外周血液中存在显著差异表达的lncRNA,可作为老年PD患者出现重度以下ARHL的潜在生物标志物。Objective To report characteristics of mild to moderate age-related hearing loss in elderly patients with Parkinson's disease(PD)and expression of plasma lncRNA.Methods Fifteen patients with mild to moderate ARHL who visited the PLA General Hospital from January 2022 to May 2023 were included(13 PD and 2 non-PD patients).All 15 patients completed audiometry tests.Whole transcriptome RNA sequencing was completed in PD patients.Auditory characteristics and plasma lncRNA expression were analyzed based on the degree of hearing loss,gender and age.Results The average hearing threshold showed no significant difference between male and female patients(P>0.05),or between the 60-69 years and 70-79 years age groups(P>0.05).In PD patients,the detection of ABR waveⅤwas the highest and wave I the lowest,although there was no significant difference in the detection of wavesⅠ,ⅢandⅤamong patients of the same gender(P>0.05).In patients with PD,RNA-seq showed 472 differences in lncRNA expression between patients aged 70-79 years and those aged 60-69 years(upregulated in 239 and downregulated in 233);535 expression differences were noticed between female and male patients(upregulated in 458 and downregulated in 77);550 differences between patients mild hearing loss and those with moderate loss(upregulated in 107 and downregulated in 443);and 1115 differences between patients with moderate hearing loss and those with moderate-severe loss(upregulated in 226 and downregulated in 889).Conclusions While there is no significant difference in audiological characteristics among PD patients regarding their levels of hearing loss,gender and age,lncRNA expression in peripheral blood can vary and therefore can potentially be used as a biomarker for diagnosis of mild to moderate age-related hearing loss in elderly patients with PD.
