益气健脾抗癌方改善肠癌化疗患者恶液质状态疗效分析

摘　　要：目的研究益气健脾抗癌方改善晚期肠癌化疗患者恶液质状态的临床疗效。方法采用随机数字表法将60例晚期肠癌应用卡培他滨方案化疗的恶液质期患者,随机分为两组,对照组及试验组各30例。对照组患者给予常规营养支持治疗(肠外营养及肠内营养),试验组在对照组治疗基础上给予益气健脾抗癌方治疗,对两组患者治疗前后的总体状态[体质量(BW)、红细胞(RBC)、血红蛋白(Hb)]、营养状态[血清总蛋白(TP)、血清白蛋白(Alb)、血清前白蛋白(PA)]、恶液质相关指标[C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]及卡氏功能状态(Karnofsky performance status,KPS)评分等数据进行统计对比。结果与治疗前相比,治疗后两组患者的体质量及RBC、Hb、TP、Alb、PA水平均有提升,且试验组高于对照组(P<0.05);与治疗前相比,治疗后两组患者的CRP、TNF-α、IL-6水平均下降,且试验组低于对照组(P<0.05);与治疗前相比,治疗后两组KPS评分升高的患者均有增多,且试验组多于对照组(P<0.05)。结论益气健脾抗癌方能显著提高晚期肠癌化疗患者的总体状态及营养状态,降低恶液质相关指标,增加KPS评分,改善其恶液质状态的疗效显著。Objective To investigate the clinical efficacy of Yiqi Jianpi Kang’ai Decoction(益气健脾抗癌方)in improving the cachexia state of patients with advanced colorectal cancer undergoing chemotherapy.Methods A random number table method was used to randomly divide 60 patients with advanced colorectal cancer who were treated with capecitabine during the cachexia status into two groups,with 30 patients in the control group and 30 patients in the experimental group.The control group was given routine nutritional support treatment(parenteral nutrition and enteral nutrition),while the experimental group was treated with Yiqi Jianpi Kang’ai Decoction on the basis of the treatment in thecontrol group.The general status[body weight(BW),erythrocyte(RBC),hemoglobin(Hb)],nutritional status[total serum protein(TP),serum albumin(Alb),serum prealbumin(PA)],related indicators of cachexia[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]and Karnofsky performance status(KPS)scores of the two groups of patients before and after treatment were statistically compared.Results Compared with that before treatment,BW,RBC,Hb,TP,Alb,and PA in both groups of patients after treatment increased,and the experimental group was higher than the control group(P<0.05);Compared with that before treatment,the levels of CRP,TNF-α,IL-6 in both groups of patients after treatment decreased,and the experimental group was lower than the control group(P<0.05).Compared with before treatment,there was an increase in patients with increased KPS scores in both groups after treatment,and the experimental group had more patients than the control group(P<0.05).Conclusion Yiqi Jianpi Kang’ai Decoction can significantly improve the general status and nutritional status,reduce indicators related to cachexia,increase KPS scores of patients with advanced colorectal cancer undergoing chemotherapy and have a significant therapeutic effect on improving their cachexia status.
