三甲散对糖尿病心肌病大鼠心肌纤维化的影响

摘  要:目的观察三甲散对糖尿病心肌病(DCM)模型大鼠心肌纤维化的影响,并探讨其作用机制。方法采用STZ单次腹腔注射联合高糖高脂饲料喂养的方法构建DCM大鼠模型,将成模大鼠按随机数字表法分为模型组、中药组、西药组、miR-21抑制组、miR-21抑制对照组,每组10只;另取10只SD大鼠作为正常组。中药组灌胃三甲散水煎液6.2 g/kg,西药组灌胃二甲双胍250 mg/kg+依那普利45 mg/kg;miR-21抑制组、miR-21抑制对照组、正常组、模型组灌胃等体积蒸馏水,1次/d,持续4周;同时正常组、模型组、中药组、西药组尾静脉注射等体积生理盐水,miR-21抑制组尾静脉注射miR-21抑制剂20 mg/kg,miR-21抑制对照组尾静脉注射miR-21抑制剂阴性对照溶液20 mg/kg,2次/周,持续4周。采用HE染色观察大鼠心肌组织形态学变化,Western blot检测心肌组织Ⅰ型胶原蛋白(Collagen-Ⅰ)表达,免疫荧光染色检测心肌组织内皮细胞标志物血小板-内皮细胞黏附分子(CD31)和间质细胞标志物成纤维细胞特异性蛋1(FSP1)表达。结果与模型组比较,中药组、西药组、miR-21抑制组心肌组织病理形态改善,Collagen-Ⅰ、FSP1表达降低(P<0.01),CD31表达升高(P<0.01);中药组Collagen-Ⅰ、CD31、FSP1表达与miR-21抑制组比较差异无统计学意义(P>0.05)。结论三甲散对DCM模型大鼠心肌纤维化具有改善作用,其作用机制可能与上调CD31表达,下调FSP1表达,抑制内皮间质转化有关,还可能与抑制miR-21表达有关。Objective To observe the effects of Sanjia Powder on myocardial fibrosis in diabetic cardiomyopathy(DCM)model rats;To elaborate its mechanism.Methods A DCM rat model was constructed using STZ single intraperitoneal injection combined with high-sugar and high-fat feeding.The model rats were divided into model group,TCM group,Western medicine group,miR-21 inhibition group,and miR-21 inhibition control group using a random number table method,with 10 rats in each group;another 10 SD rats were set as normal group.TCM group was orally administered with Sanjia Powder water decoction at a dose of 6.2 g/kg,and the Western medicine group was administered metformin 250 mg/kg+enalapril 45 mg/kg by gavage.miR-21 inhibition group,miR-21 inhibition control group,normal group,and model group were given the same volume of distilled water by gavage,once a day,for 4 weeks;at the same time,the normal group,the model group,TCM group,and the Western medicine group were injected with the same amount of normal saline in the tail vein.The miR-21 inhibition group received tail vein injection of miR-21 inhibitor at a dose of 20 mg/kg,and the miR-21 inhibition control group received tail vein injection of miR-21 inhibitor NC at a dose of 20 mg/kg,twice a week,for 4 weeks.The morphological changes of myocardial tissue were observed using HE;the expression of Collagen-Ⅰin myocardial tissue was detected using Western Blot;the expressions of endothelial cell marker CD31 and stromal cell marker FSP1 in myocardial tissue were detected using immunofluorescence method.Results Compared with the model group,the pathological morphology of myocardial tissue improved in the TCM group,Western medicine group,and miR-21 inhibition group,while the expressions of Collagen-Ⅰand FSP1 decreased(P<0.01)and the expression of CD31 increased(P<0.01);there was no statistically significant difference in the expressions of Collagen-Ⅰ,CD31,and FSP1 between the TCM group and the miR-21 inhibition group(P>0.05).Conclusion Sanjia Powder can improve myocardial fibro
