基于中土五行理论从脾胃论治妊娠恶阻

摘　　要：妊娠恶阻是妊娠期妇女常见病及多发病,威胁孕产妇及胎儿健康,中医药治疗妊娠恶阻疗效显著,且安全性高。妊娠恶阻与脾胃相关,脾五行属土,位居中央,为五脏本源,脾胃为病,生化乏源,气血不足,脏腑形体经络官窍功能失养,脾胃失和,气机逆乱;津液输布障碍,痰湿内生,阻遏中焦气机,失于通降;升降失常,冲气上逆,胃失和降,心肝肺肾之间亦可相互影响。谭枚秀教授结合临床经验,认为调理脾胃是治疗妊娠恶阻的关键。四君子汤为治疗脾胃气虚证的基础方,脾胃健运则痰湿气滞自除,调理气机则气顺痰消,谭枚秀教授以此治疗妊娠恶阻临床疗效显著。Pernicious vomiting is a prevalent and recurrent condition in pregnant women,posing a threat to the well-being of both the expectant mothers and their fetuses.Traditional Chinese medicine offers an effective and safe approach for managing Pernicious vomiting.The spleen and five elements are associated with the earth element,situated at the center,serving as the foundation for the five zang organs.When there is disharmony between the spleen and stomach due to deficiency in biochemical sources,inadequate qi and blood supply occur,resulting in dysfunction of zangfu organs'body meridians.This imbalance disrupts normal qi activity within the body,leading to disorders in fluid transportation along with endogenous phlegm dampness accumulation that hinders proper functioning of vital energy flow.Furthermore,this disorder affects not only digestion but also other organ systems such as heart,liver,lung,and kidney through disturbances in ascending-descending movements or reversed qi impulses.Based on Professor Tan Meixiu's clinical experience,regulating spleen-stomach function plays a pivotal role in treating Pernicious vomiting effectively.Sijunzi decoction serves as a fundamental prescription for addressing spleen-stomach qi deficiency syndrome by promoting healthy movement of these organs to eliminate phlegm dampness while restoring smooth qi mechanism.
