中西医结合治疗痰湿蕴脾型老年代谢综合征42例临床观察

摘　　要：目的:探讨中西医结合治疗痰湿蕴脾型老年代谢综合征的临床效果。方法:选取本院88例痰湿蕴脾型老年代谢综合征患者为研究对象,用随机数字表法分为对照组与观察组各44例。对照组给予常规西医治疗;观察组在常规西医治疗同时联用二陈汤加味。比较两组治疗前后中医症状积分、血糖指标[空腹血糖(FBG)、餐后2小时血糖(2 h PG)]、血脂指标[甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)及高密度脂蛋白胆固醇(HDL-C)]、收缩压、舒张压及体质量指数变化,统计临床疗效及不良反应发生情况。结果:对照组剔除中途退出1例,失访1例,治疗依从性差1例,最终纳入41例;观察组剔除中途退出1例,失访1例,最终纳入42例。治疗后,观察组中医症状积分、FBG、2 h PG、TG、TC、LDL-C、收缩压、舒张压及体质量指数均低于本组治疗前及对照组(P<0.05),HDL-C高于本组治疗前及对照组(P<0.05);观察组患者总有效率为95.24%,高于对照组的78.05%(P<0.05);观察组不良反应发生率为11.90%,对照组为17.07%,差异无统计学意义(P>0.05)。结论:二陈汤加味联合常规西医治疗痰湿蕴脾型老年代谢综合征可缓解患者症状,改善代谢紊乱,降低体质量指数,提升疗效,且不会增加不良反应发生风险。Objective To explore the clinical of syndrome of phlegmdamp amassing in spleen metabolic syndrome in the elderly treated by combination of traditional Chinese and Western medicine.Methods A total of 88 elderly patients with syndrome of phlegmdamp amassing in spleen metabolic syndrome in the elderly in our hospital were selected as the study objects,and were divided into control group and observation group with 44 cases in each group by random number table method.The control group was treated with conventional Western medicine,while the observation group was treated with Erchen decoction modified.Traditional Chinese medicine symptom score,blood glucose index[Fasting blood glucose(FBG),2-hours postprandial glucose(2h PG)],blood lipid index[Triglycerides(TG),Cholesterol(TC),Lowdensity lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C)],systolic blood pressure,diastolic blood pressure and body mass index were compared between the two groups before and after treatment,and the clinical efficacy and occurrence of adverse reactions were analyzed.Results In the control group,1 case was excluded,1 case was lost to follow-up,1 case was poor treatment compliance,and 41 cases were included.The observation group excluded 1 case of dropout and 1 case of missing following-up,and finally included 42 cases.After treatment,traditional Chinese medicine symptom score,FBG,2h PG,TG,TC,LDL-C,systolic blood pressure,diastolic blood pressure and body mass index of the observation group were lower than those before treatment and those in the control group(P<0.05),and HDL-C was higher than those before treatment and that in the control group(P<0.05).The total effective rate of the observation group was 95.24%,which was higher than 78.05%of the control group(P<0.05).The incidence of adverse reactions was 11.90%in the observation group and 17.07%in the control group,with no statistical significance(P>0.05).Conclusion Erchen decoction modified combined with conventional Western medicine treatment of syndrome of phleg
