温阳通脉汤治疗慢性心力衰竭的临床效果及对患者预后的影响

摘　　要：目的分析温阳通脉汤治疗慢性心力衰竭的临床效果及对患者预后的影响。方法选择2021年1月至12月收治的76例慢性心力衰竭患者纳入本次研究,随机将其分为常规组和观察组,每组38例。常规组给予常规药物治疗,观察组在常规组治疗基础上增加温阳通脉汤治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于常规组(P<0.05)。治疗后,观察组的左心室射血分数(LVEF)高于常规组,左心室收缩末期内径(LVESD)、左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)低于常规组(P<0.05)。治疗后,观察组的达到运动终点时间、6 min内最大步行距离长于常规组,终点ST段压低>1 mm占比、最大心率低于常规组(P<0.05)。治疗后,观察组的8-羟基脱氧鸟苷(8-OHdG)、丙二醛(MDA)水平低于常规组,超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)及谷胱甘肽(GSH)水平高于常规组(P<0.05)。结论温阳通脉汤治疗慢性心力衰竭患者可有效改善心功能与运动耐力,并可调节血清氧化应激指标,值得临床应用推广。Objective To analyze the clinical effect of Wenyang Tongmai decoction in the treatment of chronic heart failure and its influence on the prognosis of patients.Methods A total of 76 patients with chronic heart failure admitted from January to December 2021 were included in this study.The patients were randomly divided into conventional group and observation group,with 38 cases in each group.The conventional group was treated with conventional drugs,and the observation group was treated with Wenyang Tongmai decoction on the basis of the conventional group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the conventional group(P<0.05).After treatment,the left ventricular ejection fraction(LVEF)of the observation group was higher than that of the conventional group,the left ventricular end-systolic diameter(LVESD),left ventricular end-systolic volume(LVESV)and left ventricular end-diastolic volume(LVEDV)were lower than those in the conventional group(P<0.05).After treatment,the time to reach the end point of exercise and the maximum walking distance within 6 min in the observation group were longer than those in the conventional group,the proportion of ST segment depression>1 mm at the end point and the maximum heart rate were lower than those in the conventional group(P<0.05).After treatment,the levels of 8-hydroxydeoxyguanosine(8-OHdG)and malondialdehyde(MDA)in the observation group were lower than those in the conventional group,while the levels of superoxide dismutase(SOD),total antioxidant capacity(T-AOC)and glutathione(GSH)were higher than those in the conventional group(P<0.05).Conclusion Wenyang Tongmai decoction can effectively improve cardiac function and exercise tolerance in patients with chronic heart failure,and regulate serum oxidative stress indicators,which is worthy of clinical application and promotion.
