摘  要:大动脉粥样硬化特别是大脑中动脉粥样硬化是缺血性卒中的主要病因,其中大脑中动脉狭窄程度及斑块稳定性对于缺血性卒中的早期诊断、危险分层、预防及治疗选择至关重要。高分辨率血管壁成像可清晰显示解剖,观察血管壁及管腔的情况,还可根据斑块信号的不同及强化的高低评估血管内斑块的稳定性,为大脑中动脉所致脑卒中患者提供了有力的影像价值。本文就高分辨率血管壁成像对大脑中动脉硬化所致卒中患者的诊断、危险分层、预测复发及治疗方面进行综述,并着重指出其应用价值及一定的局限性。Large artery atherosclerosis,especially middle cerebral artery atherosclerosis,is the main cause of ischemic stroke.The degree of middle cerebral artery stenosis and plaque stability are important for the early diagnosis,risk stratification,prevention and treatment of ischemic stroke.High-resolution vascular wall imaging can clearly display the anatomy,observe the condition of vascular wall and lumen,and evaluate the stability of intravascular plaque according to the different plaque signals and the enhanced level,which provides a powerful imaging value for patients with stroke caused by middle cerebral artery.This article reviews the diagnosis,risk stratification,recurrence prediction and treatment of stroke patients caused by middle cerebral artery sclerosis with high-resolution vascular wall imaging,and emphatically points out its application value and certain limitations.
