MSCT检查在结肠癌术前TNM分期诊断的影像学特点及诊断效能分析

摘　　要：目的:研究多层螺旋CT(MSCT)检查应用于结肠癌术前TNM分期的影像学特点及诊断效能。方法:回顾性分析2020年1月—2024年5月盐城市大丰人民医院结肠癌患者100例临床资料,均实施MSCT检查,分析影像学特点以及诊断准确率。结果:病变部位以升结肠为主(43.00%),病变类型以溃疡型为主(60.00%),病变表现主要为肠壁增厚、肠腔肿块,多数邻近脂肪间隙模糊,强化程度主要为中度。T分期总准确率为74.00%,Kappa值=0.638,一致性较好;N分期总准确率为85.00%,Kappa值=0.702,一致性较好;M分期总准确率为96.00%,Kappa值=0.951,一致性非常好。结论:针对结肠癌患者,实施MSCT检查,其影像学特征明显,术前T、N分期诊断价值较高,M分期更高。Objective To study the imaging characteristics and diagnostic efficacy of multi-slice spiral CT(MSCT)examination applied to preoperative TNM staging of colon cancer.Methods Retrospectively analyze the clinical data of 100 cases of colon cancer patients in Yancheng Dafeng People's Hospital from January 2020 to May 2024 were analyzed retrospectively.All patients were implemented MSCT examination,and the imaging characteristics as well as the accuracy rate were analyzed.Results The lesion site was mainly ascending colon(43.00%),the lesion type was mainly ulcerative(60.00%),the lesion manifestation was mainly thickening of intestinal wall,intestinal luminal mass,most of the neighboring fat interstitial blurring,and the degree of enhancement was mainly moderate.The total accuracy of T staging was 74.00%,and the Kappa value=0.638,which indicated a better consistency;the total accuracy of N staging was 85.00%,and the Kappa value=0.708.Kappa value=0.702,indicating good consistency;M staging had an overall accuracy of 96.00%and a Kappa value=0.951,indicating very good consistency.Conclusion For patients with colon cancer,the implementation of MSCT has obvious imaging features,high diagnostic value of preoperative T and N staging,and higher M staging.
